SUMMARY:The Family Nurse Practitioner (FNP) at the Children’s Specialty Hospital (CSH) Program at the Center for Discovery plays a critical role in delivering comprehensive, compassionate, and specialized care to children with complex medical needs, including intellectual and developmental disabilities, autism spectrum disorder, genetic syndromes and co-occurring behavioral and medical conditions.  As a key member of an interdisciplinary team, the FNP provides advanced clinical assessment, diagnosis, and management of acute and chronic medical conditions. This includes collaborating closely with physicians, nurses, behavioral specialists, therapists, and caregivers to ensure an integrated, patient-centered approach that aligns with the individualized care plans of each child. PURPOSE: The purpose of the FNP at the CSH is to lead the medical care team to provide and deliver advanced medical care to children with complex developmental, behavioral, and medical needs. The FNP’s role is integral to the interdisciplinary care plan FUNCTIONS AND RESPONSIBILITIES:  Provides ongoing medical management for acute and chronic conditions including but not limited to; epilepsy, gastrointestinal disorders, neurodevelopmental delays, and complex behavioral challenges.  Conducting comprehensive health assessments, diagnosing medical conditions, and developing individualized treatment plans in collaboration with attending physicians and other specialists. Monitoring medication regimens and assessing for both therapeutic effectiveness and adverse effects, especially in the context of polypharmacy and behavioral pharmacology.  Supporting behavioral and developmental goals by ensuring medical issues are not contributing to behavioral presentations and working closely with psychology team.  Participating in interdisciplinary care planning, including family meetings, care conferences, and team reviews, to align medical strategies with behavioral, therapeutic, and educational objectives. Provide patient-centered care that takes into account communication barriers, sensory needs, and developmental differences.  Contributing to pre-admission, admission and discharge planning.  QUALIFICATIONS:  A Master of Science / DNP in Nursing  PMH board Certification through ANCC or AANP with 2 years of additional specialized training in psychiatric work with children, adolescents, and their families Experience with Autism and Developmental Disabilities  A current and valid NYS medical license. Valid and current D.E.A. certification. Compliance with all state and discipline-specific standards and regulations, including CPSL training and current BLS certificate.  Valid Child Abuse (Act 33), Criminal History, and FBI Clearances, CPSL training certificate.
