Ensure necessary materials and resources are prepared to deliver effective and tailored therapy sessions.- Patient Home Visits: Conduct therapy sessions in patients' homes, focusing on individualized treatment plans that address communication, cognitive, and swallowing challenges. Assess patient progress, make adjustments to therapy goals as needed, and document results to ensure measurable improvements.- Interdisciplinary Collaboration: Mid-day, communicate with the interdisciplinary care team, including nurses, physical therapists, occupational therapists, and social workers, to discuss patient progress and coordinate care plans. Ensure seamless and cohesive care delivery by aligning on shared goals.- Afternoon Documentation and Follow-Ups: Complete detailed, accurate, and timely documentation of patient sessions in the electronic health record (EHR) system. Follow up with patients and families to answer questions, provide guidance on exercises or activities, and ensure compliance with therapy recommendations.- Patient and Family Education: Provide education and training to patients and caregivers on communication strategies, swallowing exercises, and cognitive improvement techniques. Empower them to support therapy goals between visits.- Evening Reflection and Planning: Conclude the day by reviewing patient progress, identifying areas for improvement, and preparing for the next day's therapy sessions. Reflect on therapy outcomes to enhance care delivery and effectiveness.Your Expertise- Education: Master's degree in Speech-Language Pathology from an accredited program. Must hold an active state licensure or certification (CCC-SLP preferred).- Experience: At least 2 years of experience in speech therapy, with at least 1 year in a home health or similar setting preferred. Demonstrated ability to manage a diverse caseload effectively.- Clinical Skills: Expertise in evaluating, diagnosing, and treating communication, cognitive, and swallowing disorders. Proficiency in evidence-based therapy techniques and individualized care planning.- Communication: Exceptional interpersonal and communication skills to engage patients, families, and team members effectively.
