What You’ll Do (Your Impact) Build and maintain strong referral relationships with hospital case managers, discharge planners, physicians, and community partners Conduct clinical and financial evaluations to ensure appropriate patient placement Coordinate smooth, timely transitions from hospital to The Medical Suites at Oak Creek Support and guide patients and families throughout the discharge and placement process Serve as a trusted, knowledgeable resource to hospital partners Collaborate closely with our Admissions, Nursing, and Therapy teams Represent The Medical Suites at Oak Creek with professionalism, warmth, and integrity What We’re Looking For Experience as a Hospital Liaison, Clinical Liaison, Healthcare Marketer, or SNF Admissions Specialist (preferred) Strong working knowledge of Medicare, Medicaid, managed care, and discharge planning Excellent communication, relationship-building, and customer-service skills Ability to work independently within hospitals and other healthcare settings Passion for connecting patients with the right level of post-acute care and rehabilitation Why You’ll Love Working With Us Competitive salary + bonus potential Comprehensive benefits package Generous PTO program Supportive and responsive leadership Mileage reimbursement and local travel support A chance to represent a highly reputable, patient-centered facility Real opportunities for career advancement within a growing healthcare organization Be the Difference.
