Job Title: LPN Team Case Assistant STATUS: Full TimeLOCATION: Rochester, NYDEPARTMENT: Home CareSCHEDULE: DaysSUMMARYThe LPN Team Case Assistant will utilize clinical experience, and coordinate patient services under the direction of the team manager/designee. The Rochester Regional Health Home Care team has provided compassionate, personalized care and assistance for adults and children for more than 50 years, and our quality of care has been among the top-ranked home care groups in the nation.ATTRIBUTESGraduate of an accredited school of licensed practical nursing, required A valid NYS license as a Licensed Practical NurseProficient use of Microsoft OfficeOne year of experience in home care setting or medical office preferredRESPONSIBILITIES Customer Service.Manages multiple phone calls in a timely and professional manner providing appropriate follow up to assure callers’ needs are met. Validates supplemental orders and processes per policy.Supports Field Staff.Provides field staff assistance and resources for appropriate day to day issues as they arise under the direction of the team manager/designee. Remains available to assist in other duties deemed appropriate to the scope of practice of an LPN.Program Review.Collects, organizes and reviews various programs for information and reporting and initiates follow up.Discharges.Performs functions related to transfers or discharges as required by regulation.Clerical Work.Performs general office duties/projects including but not limited answering phones, documentation/order processing, copying, filing, mailing, and obtaining/ordering office supplies.Orientation.Assists in orientation of new staff, role modeling, participates in quality assurance activities.Required Qualifications:• A valid NYS license as a Licensed Practical Nurse.• Valid NYS Driver’s LicensePreferred Qualifications:• 1 year of nursing experienced preferred.• Prior home health, clinical and direct patient care experience preferred.HOW WE CARE FOR YOUAt Rochester Regional Health, we are dedicated to getting health care right.
