Job Description and HoursWhat is a LPN Case Manager?As the LPN Case Manager, you will oversee the implementation of nursing processes for individuals served within a regional area. You will work alongside the RN Case Manager supporting a caseload of approximately 200 individuals, traveling throughout the area to train staff, administer routine healthcare activities, and document reviews and medical issues.Unlock your potential with Duet. About Us: At Duet (formerly ENCOR), we provide support services to adults with intellectual and developmental disabilities with a focus on empowerment and independence throughout 5 counties in Eastern Nebraska. We assist in the activities of daily living, provide community engagement opportunities, job coaching and life skills training, as well as skilled nursing care as needed.A Day in the Life:Flexible Schedule (Monday through Friday and on-call as needed)Schedule nursing reviews ensuring the needs of the individuals are metAudit medical documentation completed by the Direct Support Professionals and ManagersAdminister routine healthcare activities and special care proceduresDocument reviews and medical issues, reporting any change of statuses to the RN Case ManagerAct as a medical liaison and medical consultantTravel up to 50% of the time throughout the specified region (North/Central Omaha) checking in on those servedReview appointment records, labs, and diagnostic testsFor the safety of those we serve, we ask the following:You possess a strong work ethicYou have 2 years of experience working as an LPNExperience with case management preferred, but not requiredPossess a valid LPN license in the state of NebraskaExperience working with the ID/DD populationKnowledge of or experience with Therap preferredMust be willing to undergo a background check, complete a physical, and a drug screenAbove all, have a passion for our missionWhat Makes us Special:Truly rewarding nonprofit experience in health & human services fieldOn-The-Job (OJT) training100% employer paid single coverage health insurance (medical, dental, vision)Casual Dress12 days Paid Time Off (PTO)11 paid holidaysPension & Retirement ProgramsTuition Reimbursement
