You will:Verify payer and reimbursement eligibilityIdentify and coordinate appropriate care team membersSchedule all visits in line with discipline and frequency requirementsKey ResponsibilitiesReview and screen incoming referrals to ensure they meet admission criteriaContact referral sources to gather required medical documentationConduct intake interviews with clients and familiesEnter accurate and timely data into the electronic medical record systemTrack client progress and satisfaction, providing follow-up support when neededQualificationsExperience in a healthcare office, preferably in home healthFamiliarity with medical and insurance terminologyIntake or admissions experience strongly preferredClear and professional written and verbal communication skillsStrong EMR and computer skillsPositive, team-oriented attitudeExcellent time management and prioritizationMilitary Occupational Classification (MOC) CodesWe value your service and skills.
