Technical Skills: Proficiency in EHR systems and telehealth platforms.Soft Skills: Strong communication, empathy, and commitment to patient-centered care.Key Responsibilities:Provide comprehensive primary care services, including diagnosis, treatment, and management of acute/chronic conditions.Develop personalized care plans emphasizing preventive health and chronic disease management.Collaborate with specialists to ensure coordinated, holistic patient care.Participate in telehealth triage services during designated weekends.Maintain accurate and timely documentation in compliance with practice standards.Mentor support staff and contribute to quality improvement initiatives.
