Key Responsibilities The Social Worker plays a critical role in ensuring residents achieve their highest practical level of well-being. Responsibilities include: Psychosocial Assessment & Care Planning: Conducting comprehensive psychosocial assessments upon admission and ongoing, identifying residents' social, emotional, mental, cultural, and spiritual needs. Collaborating with the interdisciplinary team to develop, implement, and evaluate individualized care plans. Counseling and Support: Providing individual, family, and group counseling and support for issues related to adjustment to long-term care, illness, depression, anxiety, grief, and end-of-life care. Advocacy and Rights: Advocating for residents' rights, preferences, and needs, and ensuring compliance with federal, state, and ethical standards (e.g., resident rights, elder abuse prevention). Discharge Planning: Coordinating timely and appropriate discharge planning for residents in the Transitional Rehabilitation Units (TRUs), connecting residents and families with necessary community resources and post-acute care services (e.g., home care, durable medical equipment). Resource Coordination: Maintaining extensive knowledge of and connecting residents and families to local, state, and federal assistance programs (e.g., Medicare, Medicaid) and community resources. Communication & Mediation: Facilitating communication and serving as a mediator between residents, families, and staff to resolve conflicts and address concerns. Providing education to families on medical decisions and advance directives. Documentation: Providing accurate, timely, and appropriate documentation of all social work interventions in the medical record. Qualifications Education:  Bachelor's degree in Social Work, BSW, with relevant experience in some settings Experience: Minimum of one year of experience in a healthcare or geriatric setting (e.g., skilled nursing, hospital, or long-term care) is strongly preferred. Licensure: Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in the state of Pennsylvania is highly preferred. Clearances: Must obtain all required clearances, including Act 33, Act 34, and the Older Adults Protective Services Act (OAPSA) clearance. Skills: Strong clinical, counseling, and crisis intervention skills. Excellent interpersonal, communication, and organizational skills. Knowledge of reimbursement systems and regulations related to post-acute care (Medicare/Medicaid). Ability to work effectively as part of a multidisciplinary team.
