Together, we can make a significant difference in the lives of children and contribute to a brighter and healthier future for all.Summary:The Transplant Clinical Specialist Pharmacist offers support for liver and kidney transplant services. Moreover, the Transplant Clinical Pharmacy Specialist is responsible for ensuring the delivery of safe, efficient and cost-effective pharmacotherapy in collaboration with other clinical pharmacists, coordinators, physicians and advanced practice practitioners throughout the transplant continuum (pre, peri and post-transplant phases).Responsibilities:Serves as the primary pharmacy resource for pediatric liver and kidney transplant teamsConduct comprehensive medication management and optimize immunosuppressive therapyEvaluating drug therapy impacted by the selection of immunosuppressive agents or other medications associated with transplant pharmacotherapy within inpatient and ambulatory transplant servicesMonitoring pharmacokinetic parameters of immunosuppressive agentsProvide patient and family education on medication adherence and safetyParticipate in multidisciplinary rounds and collaborate with physicians, nurses, and other healthcare professionalsDocumenting transplant pharmacotherapy plans within the electronic health recordParticipating in the evaluation of patients for potential liver and/or kidney transplantCreating and implementing guidelines and policies for optimal medication useManaging drug formulary specific to transplant servicesParticipating in quality assurance programs to enhance transplant pharmaceutical careOverview and Opportunities:Manages pharmacotherapy for transplant patientsEducates and trains pharmacy staff and other health professionalsAssists in the development of transplant program-specific protocols and guidelinesResearch for publication in peer-reviewed literaturePrecepts pharmacy students and residents as well as medical residents and fellowsPresents at local, regional, state, and national conferences QualificationsDoctor of Pharmacy (PharmD) from an accredited College or School of PharmacySuccessful completion of ASHP-accredited PGY1 residency required; PGY2 in Solid Organ Transplant (SOT) or Pediatrics strongly preferred (3 years of experience in Solid Organ Transplant may substitute PGY2 SOT residency)Licenses and CertificationsCurrent Texas state pharmacist license; eligibility for licensure in TexasSterile Compounding certificate required within 90 daysEligible for Board Certification in Solid Organ Transplant (BCTXP)BCTXP required within 2 years of employment A Place Where You BelongWe put our people first.
