This role plays a vital part in supporting residents, families, and interdisciplinary teams to ensure smooth transitions of care and exceptional resident outcomes.What You’ll Love:Competitive hourly payPaperless, quick onboarding Comprehensive benefits (health, dental, vision, retirement plan)Paid time off, holiday payGenerous employee referral programAnnual performance reviews and competitive bi-annual pay reviewsSupport for personal well-being (EAP programs, mental health resources)Respectful, supportive leadership and strong focus on teamworkOptimal staff-to-patient ratios to prevent burnoutOpportunities for advancementTuition reimbursement, training programs, and CEU supportStrong mentorship and orientation programsStrong school partnerships 360 communications for staff, residents & families Responsibilities:● Plan, coordinate, and lead interdisciplinary care plan meetings with patients, families, and community supports● Provide timely and accurate completion of the Social Services portion of the MDS, Resident Assessment Protocols, Resident Care Plan, and Progress Notes as well as Social Services Assessment, and other forms as required by the Administrator in order to comply with federal and state regulations and facility policies and procedures.● Coordinate necessary community resources upon discharge, including home health, transitional care, and meal services● Evaluate, coordinate, and facilitate access to acute psychiatric services as clinicallyindicated.● Utilize crisis intervention skills to deescalate conflicts and support patients and families during high stress situations● Monitor patient progress during skilled rehabilitation to determine readiness and ability for safe discharge home● Collaborate with the care team to identify discharge barriers and develop individualized solutions● Order durable medical equipment (DME) to support safe mobility and home use after discharge● Issue and explain Notices of Medicare Non-Coverage (NOMNCs) in accordance with Medicare guidelines and facility policy● Submit patient NOMNC appeal documentation to QIO provider in a timely manner to ensure process is not delayed● Educate patients and families about discharge options, post-acute care levels, and coverage/benefit implications● Communicate regularly with insurers, managed care organizations, and case managers regarding authorization and discharge plans● Ensure documentation of discharge planning activities in the medical record inaccordance with regulatory and facility standards● Arrange transportation at discharge, including non-emergent medical transport when needed● Facilitate placement in alternative levels of care when appropriate (ALF, LTC, memory care, inpatient psych, etc.)● Participate in daily/weekly interdisciplinary team meetings and patient care conferences● Advocate for patient rights, safety, and preferences while balancing regulatory and payer requirements● Support families and caregivers in care transitions, including scheduling caregiver support, food, transportationQualifications:Bachelor’s or Master’s degree in Social Work (BSW or MSW) requiredCurrent Social Work licensure/certification per state requirementsPrevious experience in a skilled nursing or healthcare setting preferredStrong knowledge of discharge planning, case management, and resident rightsExcellent communication, leadership, and organizational skillsCompassionate, resident-centered approach to careSalary/ Wage RangeCompensation for the role will depend on several factors, including a candidate’s qualifications, skills, competencies and experience, and may fall outside of the range shown.Your next shift could change a life.
