Recovery Case Manager

Description Full Time - Clinical Case Manager At Granite Recovery Centers, we do things differently. We celebrate being an Employer of Choice committed to providing a recovery friendly and equal opportunity workplace.  The Clinical Case Manager will assist drug and alcohol counselors with Clinical responsibilities.  These duties include and are not limited to intake, initial assessment, initial treatment planning, administrative tasks/projects, discharge summaries, individual assessments with clients and consultation with other professionals in regard to client treatment and services.  The Clinical Case Manager will work effectively and cooperatively with the Clinical staff to support clients to abstain from substances and live happy and productive lives free of their addition.  Qualifications: Bachelor’s degree in related field required Direct experience working in field of mental health and substance use disorders Versatile, flexible, and willing to work within constantly changing priorities with enthusiasm Time management and organization with high efficiency in a fast paced, dynamic organization Must meet pre-employment and maintain all applicable state, national, and job-related guidelines for reference checking, background screening,10 panel urine drug screening, health screening, DMV (if applicable), insurance (if applicable), and license/credential verifications Responsibilities: Complete biopsychosocial for admitting clients within 24 hours of admission as directed by Assistant Clinical Director or Clinical Director Create and complete initial master treatment plan within 48 hours of client’s admission to facility Obtain client signature for master treatment plan within 24 hours of treatment plan creation Assist with interagency records transfer to Clinical Supervisors of each designated program and adhere to the procedure in excellence Assist with documentation of 50 day follow up phone calls for clients who have discharged over 60 days from current calendar day as assigned by Clinical LeadershipAssist with administration tasks and/or projects including but not limited to: Schedule staff o Track and audit group notes Document for efficiency within the company Collaborate with Clinical Assistants to track ASAM documents Able to effectively communicate with people at all levels and from various backgrounds  Provide daily communication/collaboration with treatment team of residents entering treatment Meet with supervisor on a regular schedule for continued education and supervision of job responsibilities Adhere to all HIPAA guidelines while employed at facility Participate in continuing education to continually improve skills and abilities and stay abreast of current technologies/practices Benefits: Competitive salaryComprehensive benefits package including medical, dental, vision and 401(K)Generous paid time off accrualExcellent growth and development opportunitiesSatisfying and rewarding work striving to overcome the opioid epidemic Here is what you can expect from us: Granite Recovery Center is committed to helping people achieve lasting sobriety through comprehensive care and compassionate support.
