Nurse Aide I
NA I provide Level I-Level III Care
May perform 3 or more Activities of Daily Living (ADL'S)
Mission Statement: To provide services, intergenerational activities, and opportunities to the elderly and disabled adults of Ashe County that enhance their dignity and help them maintain their independence at home.
Desirable Education and Experience/RequirementsGraduation from high school and some experience as a caregiver, non-certified aide, or an NA I in a nursing home, home care agency, hospital or an equivalent combination of education and experience. Must hold a current N.C. unencumbered Nurse Aide I Certificate issued by the Division of Facility Services (Guidelines/Regulations per the NA I and Health Care Personnel Registry). Possession of a valid N.C. driver's license. A reliable vehicle for travel to and from client homes.
General Statement of NA I DutiesWork involves personal care (ADL'S) to include, but not limited to, bathing, dressing (includes grooming), ambulation/transfers (mobility), eating, toileting, bowel/bladder incontinence and home management (house cleaning) in the client's home (older adults, disabled persons and children), essential errands for client under the supervision of an appropriate professional/paraprofessional.
Distinguishing Features of the NA I PositionNA I provide the above-mentioned general duties. Socialization, community inclusion and caregiver respite may be included per client need. An employee may work with a client for a short time frame or for an extended period of time depending on client needs and staffing. Assignments may change frequently. Duties and percentage of time spent in various activities may change based on client needs. Work includes written documentation utilizing progress notes, time sheets and mileage sheets. Progress notes/time sheets reflect, but are not limited to, observations of client needs, changes in client condition, tasks completed, etc. Work is performed according to NA I regulations, per the client Plan of Care (POC), and as directed by a Supervisor/RN. Duties may include, but are not limited to the following activities: assist with feeding clients with special conditions/swallowing difficulties; make an occupied bed; assist with mobility, gait training, transfers using assistive devices; assist clients with ROM exercises; provide care to clients requiring extensive assist with ADL'S; assist with purchasing/preparation/planning of specific diets (diabetic, low salt, etc). Duties may expose the employee to human body fluids and thus work is subject to OSHA Requirements for Blood-Borne Pathogens. Work is supervised by In-Home Aide Supervisor (RN), and is reviewed through conferences; review of records/reports, observation and from feedback from client/family/caregiver and case managers.
Must be able to complete a Competency/Skills Checklist as reviewed/directed by an RN with Generations Ashe.
Other Job DutiesPerforms related duties as required.
Physical RequirementsMust be able to physically perform basic life operational functions of climbing, balancing, stooping, kneeling, crouching, reaching, standing, walking, pushing, pulling, lifting, grasping, talking, and hearing.
Must be able to perform heavy work exerting up to 100 pounds of force occasionally, and/or up to 50 pounds of force frequently, and or up to 20 pounds of force constantly to lift objects.
Must possess the visual acuity to operate a motor vehicle, to make visual interpretations of client and home conditions, and to determine the neatness and thoroughness of the work performed
Recruitment and Selection GuidelinesKnowledge, Skills, and AbilitiesSkills in listening/communicating with client/family/caregiver/supervisory staff/other employees.
Ability to read/write. Ability to communicate effectively in oral/written form.
Ability to drive a vehicle.
Ability to maintain and develop effective working relationships with staff/client/family/caregiver, supervisors, case managers, other employees and the general public.
Ability to inspect and monitor client safety issues, and report concerns to appropriate supervisor.
Ability to act as a client advocate. Provide a support base for client/family.
A. Activities of Daily Living- Provide bath, shampoo/hair care, skin care, mouth care, shaving (legs, underarms, face), dressing, incontinence needs such as diaper changes, fingernail care, perineal care. Reinforce personal hygiene and self-care.
- Assist with range of motion exercises, transfers, and use of assistive devices when directed by physician/physical therapist. Assist with client mobility.
- Take/record client temp., blood pressure, pulse, respirations, weight, blood sugar check per POC/MD orders.
- Observe/remind client to take self-administered or caregiver pre-poured medications. NA I cannot give or prepare medications including over the counter medications, medicated lotion/powder, enemas, eye drops, ointments, injections/shots. Cannot do dressing changes.
- Monitor and follow POC. Provide feedback to supervisor/case manager/nurse/peer mentor as needed.
- Reinforce housekeeping methods, home safety, energy conservation, and
sanitation.
- Proper food handling, cooking, and storage. Assists clients with meal/snack preparation, assist with feeding of client/feed client.
- Light housekeeping to include but not limited to, sweeping, trash, make occupied/unoccupied bed, clean bathroom, clean kitchen/client dishes, counter tops, microwave, toaster oven, stove and refrigerator, vacuum, dust, change linens, laundry, mop (use a container).
- Supportive Services
- Maintain progress notes daily on each client in the client's home. Maintain time sheets and mileage sheets.
- Report changes in client/family conditions/needs to appropriate supervisor. Act as an advocate for the client/elderly/disabled/handicapped.
- Assist in educating the public/client/client caregiver as to available community resources. (Information/Referral).
- Understanding HIPPHA Guidelines/Regulations to maintain client confidentiality.
- Attend required monthly training. Acquire 12 hours of mandatory training per year and maintain documentation of said training.
- Provide transportation for the client to essential services (not to medical appointments unless private pay client). Transport only the client. Have reliable means of transportation at all times.
- Provide companionship/socialization/community inclusion, care giver respite per POC and as directed by supervisory staff.
- Assist client as needed to pay utility bills.
- NA I must be able to demonstrate knowledge and skills required for
competency in all levels of tasks/care to be performed.
- Maintain high standards of personal conduct and integrity/ethics both on
and off the job.
- No conflict of interest issues.
