Healthcare Nursing of Brighton - A well-established Skilled Nursing and Rehabilitation facility located in the Brighton, NY area is seeking a dedicated and compassionate Social Worker to join our team.This role is ideal for someone who is passionate about enhancing the quality of life for older adults and thrives in a supportive, interdisciplinary healthcare environment. The Social Worker plays a vital role in ensuring residents receive the emotional, social, and psychosocial support necessary to promote dignity, independence, and overall well-being.Responsibilities include:• Assessing and supporting residents’ social, emotional, and daily living needs to help maintain or improve their psychosocial functioning.• Identifying individual concerns through ongoing interaction and documentation.• Developing, implementing, and regularly updating individualized care plans tailored to each resident’s unique needs.• Participating in admission planning, discharge planning, and community re-entry processes to ensure smooth transitions and continuity of care.• Serving as an advocate for residents and families, providing guidance, resources, and support throughout their stay.• Participating in interdisciplinary care plan meetings, collaborating with nursing, therapy, and administrative teams to ensure residents receive comprehensive, person-centered treatment.• Maintaining strong communication with families, outside agencies, and internal departments.• Ensuring compliance with all state, federal, and facility regulations related to resident rights and psychosocial care.Requirements:• Bachelor’s Degree in Human Services, Psychology, Social Work, or a related field.• Prior experience in a long-term care or subacute rehabilitation setting is required.• Strong interpersonal, communication, and organizational skills.• Ability to work collaboratively as part of a multidisciplinary team.• Compassionately serve a diverse resident population with professionalism and empathy.Compensation:$60,000-$65,000/year, and our generous benefits packageApply now to join our amazing team
