Position Summary:
The Nurse Practitioner (NP) plays a pivotal role in providing advanced wound care and feeding tube management services. This is a field-based position requiring daily travel within an assigned region to deliver care in diverse clinical environments, including skilled nursing facilities, long-term care facilities, and private residences.
The ideal candidate will demonstrate expertise in patient care, effective communication, problem-solving, and the ability to adapt to dynamic care settings. The role involves collaborating with multidisciplinary teams, upholding compliance with HIPAA and other regulations, and making recommendations to improve workflows and enhance operational efficiency.
Key Responsibilities:
1. Advanced Clinical Care
Assess, diagnose, and manage acute and chronic wounds, including venous ulcers, pressure ulcers, diabetic wounds, and surgical wounds.
Perform bedside procedures such as:
Excisional and sharp debridement
Negative pressure wound therapy management
Application of advanced wound modalities (e.g., skin grafts, ultramist)
Oversee feeding tube placements, replacements, and maintenance with a focus on safety and precision.
Monitor wound healing progress and make adjustments to care plans based on evidence-based practices.
2. Patient Advocacy and Education
Serve as an advocate for patients, addressing concerns and creating care plans tailored to their unique needs.
Educate patients, caregivers, and facility staff on:
Wound care techniques
Feeding tube management, safety, and troubleshooting
Recovery strategies and health promotion practices
Build trust, empathy, and strong relationships with patients and their support networks.
3. Professionalism and Collaboration
Exhibit professionalism in all interactions, ensuring respectful, courteous communication with patients, families, and care teams.
Collaborate with multidisciplinary teams, including physicians, other NPs, RNs, and facility staff, to ensure seamless, patient-centered care.
Contribute to a culture of integrity, accountability, teamwork, and continuous learning, supporting organizational goals.
4. Effective Communication and Problem-Solving
Communicate clearly and effectively to address patient needs, resolve concerns, and develop collaborative care plans.
Utilize active listening and empathy to build rapport and trust with patients and colleagues.
Identify barriers to care delivery or workflow inefficiencies, proposing actionable solutions to improve outcomes.
Provide concise updates to managers and stakeholders on challenges, opportunities, and areas for improvement.
5. Efficiency and Workflow Recommendations
Evaluate clinical workflows, protocols, and processes to identify areas for improvement.
Make data-driven recommendations to enhance efficiency, reduce redundancies, and optimize resource use.
Partner with management to implement improvements that promote patient satisfaction, provider efficiency, and operational excellence.
6. Documentation and Compliance
Maintain accurate, thorough documentation of all patient encounters in electronic health record (EHR) systems.
Ensure compliance with HIPAA and all state, federal, and organizational regulations governing patient care and privacy.
Participate in quality assurance activities, including audits and compliance reviews, to maintain OWP's high standards.
7. Regional Travel and Mobility
Travel extensively within the assigned region, driving up to 50–60 miles daily to visit multiple care sites.
Maintain reliable transportation, a valid driver's license, and a clean driving record.
Adapt to varied care settings, ensuring consistency and high-quality care delivery.
8. Technology and Communication Proficiency
Leverage technology tools, including:
Microsoft Office Suite (Word, Excel, Outlook)
Google Workspace (Docs, Sheets, Drive)
Collaboration platforms (Slack, Zoom, Microsoft Teams)
EHR systems and mobile healthcare devices
Troubleshoot minor technical issues to ensure workflow continuity and effective care delivery.
9. Safety and Emergency Preparedness
Follow infection control protocols, including appropriate use of personal protective equipment (PPE).
Respond promptly to urgent patient needs, including wound-related emergencies or feeding tube complications.
Report incidents, near-misses, or hazards according to organizational safety policies.
10. Organizational Commitment
Foster a culture of respect, collaboration, and innovation, contributing to a supportive workplace environment.

Qualifications:
Education & Licensure:
Graduate of an accredited Nurse Practitioner (NP) program.
Active, unrestricted NP license in California and/or Texas.
National certification through AANP or ANCC.
DEA registration to prescribe controlled substances.
Experience & Skills:
Minimum of 2 years in clinical practice (experience in geriatrics, family medicine, internal medicine, or wound care preferred).
Strong clinical judgment and proficiency in wound care and feeding tube management.
Effective communication, problem-solving, and decision-making skills.
Ability to work independently while maintaining high clinical standards.
Driving & Technology Proficiency:
Valid driver's license with a clean driving record.
Reliable transportation for daily travel (mileage reimbursement or company vehicle provided).
Proficient in Microsoft Office Suite, Google Workspace, and collaboration tools (Slack, Zoom, Microsoft Teams).
Familiarity with EHR systems and healthcare technology.
Language Requirements:
Fluent in English, with strong written and verbal communication skills to interact with patients, families, and care teams.
Physical Requirements:
Ability to stand, walk, bend, and kneel frequently during patient care activities.
Ability to lift and carry up to 20 pounds of medical equipment or supplies.
Comfortable traveling extensively within the assigned region and working in diverse environments.
