This role is a 1099 contract opportunity, with care being provided at the patients residence. Patient needs range from weekly hours to 24-hour care.This case is in Carson City and offers 6 hours a month, a great opportunity for those looking for part-time, flexible hours Once our patients are accepted into the program, there is no expiration for this benefit, which allows our team of caregivers to build lasting relationships with their patients for years to come.Youll get to advocate for the patients and work one-on-one to provide them with the care that they need. This role will collaborate with CNAs, HHAs, Case Managers, and the patients physicians. Offers are contingent upon passing a background check.ResponsibilitiesExecute physician prescribed plans of careComplete compliant documentation of the care provided in the system of recordManage and administer prescribed medication, treatment, and therapiesConduct patient assessments, coordination of carePerform various activities associated with daily livingMonitor vitals and GI intake and outputAssess skin integrity and administer wound careMonitor for changes in the patients condition such as weight loss/gain, self-care abilities, and indicators of disease progressionCommunicate with family members, physicians, case managers, and other prescribed individuals regarding the health of the patientsProvide education and training to the patient, their family, and/or the Home Health AideWork with patients to be done one-on-one in the patients home to provide for a comfortable environmentCollaborate with and supervise Certified Nursing Assistants and Personal Care AttendantsPerform other personal care services as necessary to meet the patients needsMinimum QualificationsEducation, licensing, and certification: graduate of an accredited college or university, Associate or Baccalaureate School of NursingCurrent State or Compact License as a Licensed Practical NurseTB test and physical examinationCurrent CPR certificationMaintain required insuranceGood interpersonal skillsHand-eye coordination and manual dexterityAbility to stand or walk for prolonged periods, with the ability to lift up to 50 lbs and move patientsAbility to operate under stressful conditions and make quick decisionsMust have dependable transportation and be willing to commute to the patients homeAbility to provide basic medical management up to medically fragile end of life carePreferred Qualifications1+ years of experience as an LPN in a clinical or home health setting.Ability to assist with two person assistTo provide the exceptional care our patients deserve, we rely on a team of passionate, dedicated professionals.
