Provides clinical nutrition services to participants at the Renker Wellness Center as a component of medical programs (cardiac/pulmonary rehab and/or heart failure clinic) as well as fee for service for RWC community gym members. Responsible for coordination and implementation of nutrition care with medical, nursing, and ancillary care professionals. Reports to Director, Renker Wellness Center Supervises N/A Ages of Patients Adult Geriatric Blood Borne Pathogens Minimal/ No Potential Reports to expand_more Reports to Director, Renker Wellness Center Supervises expand_more Supervises N/A Ages of Patients expand_more Ages of Patients Adult Geriatric Blood Borne Pathogens expand_more Blood Borne Pathogens Minimal/ No Potential Qualifications expand_more Qualifications Education Required: Bachelor's Degree Licensure/Certification Required: Registered dietician with Commission on Dietetic Registration (CDR) accreditation and American Heart Association BLS Experience Required: Two (2) years of experience as a Registered Dietician in a healthcare setting Preferred: Experience in a medical program model including Epic electronic health record; experience with cardiovascular disease, pulmonary disease, diabetes, weight management, and plant based nutrition Essential Responsibilities expand_more Essential Responsibilities Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations Assesses the nutritional status of adult and/or geriatric patients with clear documentation in the medical record. Uses parameters such as anthropometric measurements, visual assessment and interpretation of laboratory values Develops and revises a nutrition care plan based on information from the client, medical record, family and health care team members Incorporates current evidence-based research and practice into educational materials, documentation, and policy and procedures Performs nutrient intake analysis Adheres to facility confidentiality, HIPPA regulations, and patient rights policies Complies with regulatory agency standards, including federal, state and The Joint Commission as evidenced by successful completion of surveys with no deficiencies Completes all required documentation, reports and logs in a timely, professional and thorough manner Communicates effectively with all team members, patients and families through conferences, individual consultation and committee participation. This includes telephone, written, and electronic communication Orients, mentors and educates other staff, as applicable Advises department on nutritional care Knowledgeable of current evidence as relates to plant-based nutrition for populations served by the Renker Wellness Center Plans regular and modified menus as requested Follows facility and department infection prevention policies and procedures Participates in facility-wide and department Performance Improvement Program and on Performance Improvement teams to improve processes and care Demonstrates leadership as nutritional care expert for others on the inter-professional health care team at the Renker Wellness Center Provides community education as requested within area of expertise including support of Community Heart Failure Clinic lecture series and educational programs for cardiac/pulmonary rehab participants Assists and contributes to stated patient satisfaction goals Follows all security procedures for facility and department Performs other duties as assigned Apply Now Share Job: Email Facebook Linkedin
