Reports directly to the Activities Director (Therapy Aide Supervisor). •    Assist with developing and implementing an activities program, which ‎maintains or improves physical, mental, and social well-being with a varied work schedule that may ‎include weekend, evenings, and holidays‎•    Assist with developing an activities plan for each resident in accordance with the inter-‎disciplinary team process. Responsible for the documentation of that plan in the resident’s medical record•    Responsible for initial interview with new residents and input of resident data into the ‎medical record•    Performing duties of the Activities Director when the Activities Director is unavailable•    Responsible for maintaining the activity space and requesting of, equipment and ‎supplies for the program and ensuring resident safety for individual and/or group ‎activities‎•    Provide necessary assistance by giving residents the following Activities of ‎Daily Living nursing care: oral hygiene, ‎bathing, dressing, and grooming, hair and nail ‎care, shaving, toileting, assistance with meals, nourishments and transportation ‎to ‎meal service areas, and mobility. Routinely observe, identify and report resident ‎concerns/needs to supervisor. Provide ‎escort to resident appointments when required. ‎Complete assignments timely, completely and accurately. Accept assigned ‎duties in a ‎cooperative manner‎•    Perform restorative nursing measures. Document in the nursing assistant notes the care ‎and treatment provided to the resident ‎and the response or lack of response to care ‎provided•    Perform simple nursing measures such as but not limited to: applying splints and ‎positioning devices, making occupied and ‎unoccupied beds, changing linens, taking vital ‎signs, weights, recording intake and output, etc. Follow established performance ‎‎standards and perform duties according to nursing service policies and procedures. ‎Review care plans and perform nursing ‎care as outlined. Participate in the development ‎of an individualized plan of care of assigned residents ‎•    Respond to residents' requests/needs and assure resident have call lights at hand and ‎answer call lights promptly•    Observe all facility safety, infection control policies and procedures‎•    Complete required documentation. Consistently work cooperatively with resident, ‎resident’s representatives, facility staff, physicians, consultants and ancillary service ‎providers. Maintain confidentiality of resident.•    Attend all in-service program classes as assigned and complete assignment. ‎Participate in interdisciplinary team and other facility meetings as assigned. Follow ‎Residents Rights’ policies as all times.
