With integrity at the center of all we do, we know that when we support you and your community, you'll change lives every day.As a Hospice Medical Social Worker, you will:Provide direct patient counseling services; support crisis interventions as neededAssist patients and family in community resource planning and accessHelp the patient and family navigate their ongoing healthcare journeyTo qualify as a Hospice Medical Social Worker with us, you will need:Education: Bachelor of Science in Social Work (BSW) or comparable undergraduate degree required; Master of Social Work (MSW) preferredLicensure: Current unrestricted license to practice as a Licensed Social Worker (LSW) in the state associated with this position required; Licensed Independent Social Worker (LISW) preferredOne (1) year of experience practicing as a Licensed Social Worker in a hospice/palliative care or similar settingReliable transportation to/from care sites/ or work locationsAt Interim HealthCare Hospice & Palliative Care, we know that being our best is non-negotiable - that's why we treat your family like our own.
