A rewarding opportunity is available for a Registered Nurse specializing in Home Health care to provide compassionate, patient-centered support in the comfort of clients' homes. This contract position allows for hands-on nursing that makes a tangible difference in patients lives by promoting healing, independence, and well-being outside of a traditional hospital setting.Key Responsibilities:Deliver comprehensive nursing care to patients in their homes, including assessments, wound care, medication management, and education on health maintenance.Collaborate with interdisciplinary teams to customize care plans tailored to individual patient needs.Monitor patient progress and adjust care plans accordingly to ensure optimal outcomes.Educate patients and families on managing chronic conditions and post-hospital recovery.Maintain accurate and timely documentation of all nursing activities and patient interactions.Desired Qualifications:Valid RN licensure in the state of practice.Proven experience or interest in home health nursing.Strong assessment and clinical skills combined with excellent communication and interpersonal abilities.Ability to work independently while managing diverse patient caseloads.Compassionate approach with a commitment to patient advocacy.Location:Barre, Vermont offers a welcoming community with a combination of small-town charm and access to outdoor recreational activities.
