Family Centered Medicine is a well-established adult medicine clinic in Denver, dedicated to providing compassionate and inclusive healthcare to a diverse patient population.
Specializing in HIV/AIDS treatment, PrEP, and transgender care , we foster a supportive and welcoming environment for individuals of all backgrounds.
We are seeking a compassionate and skilled Nurse Practitioner or Physician (MD/DO) to join our team in a part-time or full-time capacity.
Position Overview As a Physician or Nurse Practitioner , you will play a vital role in improving health outcomes for adults, particularly in underserved communities.
Your focus will include preventive care, chronic disease management, and specialized services such as HIV/AIDS care, PrEP, and transgender healthcare .
You will collaborate with a multidisciplinary team to provide comprehensive, patient-centered care.
Key Responsibilities Patient Care:- Assess, diagnose, and develop personalized care plans for adult patients.
- Treat and manage conditions such as diabetes, hypertension, and asthma.
- Provide HIV/AIDS care, PrEP, and transgender healthcare services.
- Work closely with a team of healthcare professionals to ensure comprehensive care.
- Empower patients with knowledge and self-management strategies.
- Maintain accurate and up-to-date electronic health records (EHR).
- Active NP license in Colorado.
- Master's degree in Nursing and board certification.
- Active, unrestricted medical license to practice in Colorado.
- Experience or strong interest in HIV/AIDS care, PrEP, and transgender healthcare is preferred.
- Commitment to working with diverse patient populations.
- Strong communication skills and dedication to patient confidentiality.
