Spring Hill Rehabilitation & Nursing Center - We are seeking a dedicated and compassionate Occupational Therapist to join our healthcare team. The ideal candidate will provide comprehensive therapeutic services to patients across various settings, including inpatient, outpatient, home health, schools, and skilled nursing facilities. This role involves developing and implementing individualized care plans aimed at improving patients' functional abilities and quality of life. The Occupational Therapist will collaborate with multidisciplinary teams to deliver patient-centered care, assist with activities of daily living (ADLs), and support discharge planning processes.

Duties

Conduct thorough assessments of patients’ physical, cognitive, and emotional needs using physiology knowledge, medical terminology, and anatomy expertise. Develop and implement personalized care plans tailored to each patient's goals and medical condition. Assist patients with activities of daily living (ADLs), including dressing, grooming, feeding, and mobility. Provide therapy interventions for diverse populations such as pediatrics, geriatrics, individuals with autism, disabilities, or those recovering from acute care or ICU stays.

Support discharge planning by coordinating with healthcare teams to ensure smooth transitions from hospital to home or other care facilities. Engage in case management activities to monitor progress and adjust treatment plans accordingly. Collaborate with speech therapists and other specialists when necessary to address comprehensive patient needs. Participate in documentation of patient progress, treatment outcomes, and care plans in compliance with healthcare regulations.

Educate patients and families on therapeutic exercises, adaptive techniques, and safety measures for ongoing recovery or health maintenance. Maintain a safe and clean work environment adhering to healthcare standards.

Requirements

Valid occupational therapy license in the relevant state or region. Proven experience working in inpatient settings such as ICU or acute care units is preferred.

Experience

with early intervention, outpatient clinics, skilled nursing facilities, post-acute care, home health services, or school-based programs is highly desirable. Knowledge of working with people with autism, disabilities, geriatrics, or those requiring specialized therapies such as speech therapy is a plus. Strong understanding of physiology, anatomy, medical terminology, and patient care principles. Ability to develop effective care plans tailored to individual patient needs.

Excellent communication skills for collaborating with multidisciplinary teams and educating patients/families. Ability to work independently while managing caseloads efficiently across various settings.
