Job Summary:

This position is responsible for the coordination and delivery of clinical behavioral health services designed to meet the wellness and recovery needs of consumers, including services provided to individuals experiencing homelessness or housing instability. These services may include resource development, assessments, and counseling provided individually or in a group setting and may involve the consumer's family members. This position specifically targets homeless and unstably housed individuals with a focus on integrating mental health treatment and housing support to ensure long-term stability and recovery.

Essential Functions:

Intake:

Preparation of intake evaluations and diagnostic assessments, specifically for consumers experiencing homelessness or housing instability. Complete thorough assessments of mental health needs, risk factors, and housing barriers for consumers.

Clinical Services:

Provide individual, group, and family therapy to consumers with mental health challenges and housing instability, utilizing trauma-informed care. Conduct psycho-educational groups, focusing on topics related to housing stability, mental health recovery, and navigating the housing system. Collaborate with the clinical team, case managers, and housing specialists to address both mental health and housing needs of clients. Consult with psychiatrists regarding program clients as needed for medication management and treatment coordination.

Documentation:

Maintain comprehensive clinical documentation in Millennium, the agency's electronic health record system, including intake evaluations, assessments, treatment plans, progress notes, collateral contacts, and termination summaries. Document all services provided in a timely manner, ensuring compliance with agency documentation standards and requirements.

Case Management:

Provide continuous communication and outreach to clients, including follow-up for hard-to-engage individuals, with a focus on maintaining housing stability. Assist with navigating housing systems, including referrals to emergency shelters, permanent housing, and housing-first programs. Act as a liaison to other agencies involved in the 1115 Waiver program, ensuring seamless service integration and client advocacy. Provide information and referrals to community resources, including support services for mental health, housing, employment, and social services.

Productivity:

Clinicians are expected to spend 65% of their time in face-to-face consumer contact, directly engaging with clients in therapy, case management, or housing support. Participate in individual and peer supervision sessions, ensuring continuous professional development and clinical support. Provide routine productivity reports to the Program Director, ensuring compliance with program targets. Submit all schedules, statistical reports/logs, and clinical reports within prescribed timelines, ensuring adherence to program expectations and regulatory requirements.

Supervision:

Clinicians are required to participate in individual and peer supervision according to their program needs. Treatment plans may need to be done in consultation with the program psychiatrist.

Other Duties:

Represent the agency at Multidisciplinary Team meetings related to housing and mental health services. Participate in the Quality Improvement process and specific quality initiatives for the program. Interface with medical, vocational, residential and other program resources as necessary. Work as part of a team to provide services to consumers outside of primary caseload as needed. Supervise interns fulfilling field work requirements as needed. Utilize supervision and training as opportunities for enhancing professional competence. Performs all duties in a professional and ethical manner consistent with agency, licensing, COA, and professional standards. Perform other related duties as assigned.

Knowledge/Skills/Abilities:

Expertise in treating individuals with complex behavioral health challenges, including those affected by homelessness, trauma, and mental health disorders. Knowledge of the 1115 Waiver program requirements, housing-first principles, and resources available to individuals experiencing homelessness or housing instability. Strong communication and case management skills, with the ability to advocate for clients within multidisciplinary settings. Ability to work effectively with clients in crisis and those with chronic mental health or housing instability issues. Computer literate

Minimum Qualifications:

Master's Degree in Social Work (MSW) from an accredited graduate school of Social Work, or a Master's Degree (MA or MS) in Counseling. Valid driver's license. Professional licensure- Clinical license required or license-eligible (LSW, LAC). Exempt/Non-Exempt Exempt Weekly Work Hours 40 hrs Proposed Salary $65,000/annually Schedule
