*This position has a sign-on bonus of $3,000 along with a $500 referral fee for other LPN/RNs you refer for the company* Hourly rate: $26-$32/hr Position Summary: The Outpatient LPN provides intensive, community-based services to individuals with serious mental illness. The LPN plays a key role in supporting the physical and behavioral health needs of our consumers through direct care, medication management, and health education. This position is ideal for a compassionate nurse who thrives in a collaborative, field-based environment and is committed to recovery-oriented care. Key Responsibilities: Medical Oversight and Coordination Conduct health assessments and monitor vital signs during home and community visits Coordinate medical appointments and follow-up care with primary and specialty providers Assist with lab coordination and specimen collection when appropriate Maintain medication inventory and ensure proper storage and handling Ensure compliance with HIPAA, ADMH, and agency policies Document all services accurately in the electronic health record (EHR) Participate in quality improvement initiatives and required trainings Medication Management Administer prescribed medications and monitor for side effects or adverse reactions Educate consumers on medication adherence and chronic condition management Support medication reconciliation and adherence tracking Collaborate with prescribers to report concerns or changes in consumer health status Team and Consumer Support Participate in daily ACT Team meetings and contribute to individualized treatment planning Support crisis intervention and stabilization efforts in the field Provide outreach and engagement to consumers who may be reluctant to participate in services Collaborate with psychiatrists, nurses, therapists, case managers, and peer specialists Promote wellness and recovery through education and supportive care Build trusting relationships with consumers to encourage participation in services Documentation and Compliance Accurately document all clinical interactions, interventions, and observations in the electronic health record (EHR). Ensure compliance with HIPAA, OSHA, and other regulatory standards. Participate in quality improvement initiatives and audits as needed. Maintain up-to-date licensure, certifications, and required training. Qualifications: Current LPN license in the State of Alabama Nursing degree from an accredited institution. Minimum of 1–2 years of nursing experience (behavioral health or community-based care preferred) Valid driver’s license and reliable transportation required Strong communication, organizational, and interpersonal skills Proficiency in Microsoft Office and EHR systems Ability to work independently and as part of a multidisciplinary team Commitment to trauma-informed, culturally competent, and recovery-focused care Strong clinical judgment, documentation, interpersonal, and organizational skills. Ability to work independently across multiple sites. Strong communication and interpersonal skills. Ability to work with compassion, patience, and flexibility in a variety of community settings. Ability to maintain confidentiality and adhere to professional boundaries. Transportation Requirements: May provide transportation for consumers as needed using agency or personal vehicles. Maintain a safe driving record and valid driver’s license with personal liability insurance. Follow all agency transportation policies, including seatbelt usage, safe cellphone practices, and vehicle documentation. Immediately report any vehicle issues, accidents, or violations to appropriate personnel. Must have reliable transportation, maintain a safe driving record and valid driver’s license with personal liability insurance. Work Environment & Schedule: This is a non-exempt, full-time position. Standard hours are Monday through Friday, 8:00 AM – 4:30 PM CST. This position requires in-person work and is not eligible for remote or hybrid work arrangements. Requires travel between multiple community sites. Fast-paced environment involving direct interaction with consumers and staff. In-Service Training & Continued Education Requirements: Participation in all agency-required in-service trainings. Commitment to ongoing professional development to enhance skills and stay informed on best practices in behavioral health. Completion of onboarding and any continuing education requirements as directed by the agency or supervisor. Attend staff meetings, trainings, and in-service sessions. Maintain licensure and participate in ongoing education related to mental health, ID/DD care, and medication administration.
