Arc at Cincinnati - Arc at Cincinnati is looking to add a Licensed Social Worker to our team  The Licensed Social Worker provides psychosocial support to residents and families in a skilled nursing and assisted living environment. This role ensures compliance with state and federal regulations while promoting dignity, independence, and quality of life for residents. Key Responsibilities Conduct initial and ongoing psychosocial assessments for residents. Develop and implement individualized care plans in collaboration with interdisciplinary teams. Provide counseling and crisis intervention for residents and families. Facilitate discharge planning and coordinate community resources. Document social service needs and progress per regulatory requirements. Advocate for residents’ rights and assist with conflict resolution. Participate in care plan meetings and quality assurance initiatives. Qualifications Current Ohio LSW license (required). Bachelor’s degree in Social Work; Master’s preferred. Experience in long-term care or healthcare setting strongly preferred. Knowledge of Medicare/Medicaid regulations and discharge planning. Strong communication, organizational, and problem-solving skills.
