Skilled Nursing Facility - Social Services DirectorSocial Services Director Job Type:Full TimeGreat BenefitsCompetitive PayWe are seeking a full-time Social Services Director / long-term care Social Worker to provide assessment, advocacy, and discharge planning for the psychosocial needs of patients of all ages. This role requires expertise in crisis intervention, mandated reporting, and care coordination for patients with psychosocial risk factors. Responsible for education and advocacy for patients, families, and significant others.The Social Services Director coordinates the psychosocial needs of residents and families and works closely with the Nursing Department to support resident care in a long-term care / healthcare facility.Social Services Director Requirements:Bachelor’s Degree in Social ServicesCSW (Certified Services Worker) certification—State of MissouriKnowledge of community resourcesExperience in healthcare social workAbility to prioritize multiple tasksStrong interpersonal and communication skillsProfessional demeanor with patients, families, interdisciplinary teams, and outside agencies2–5 years of acute care or long-term care experience preferredMDS experience preferredLong-term care social work experience a plusWe look forward to meeting youFor questions, please call 816-237-8799.
