Northwest Habilitation Services - Location: Salem and surrounding area Schedule: 30-40 hours per week Status: Full time Pay/ Additional Benefits: Hourly Position. Pay $23 an hour. Mileage reimbursement and travel time between clients. Medical, Dental and Vision through UHC offered after 90 days of employment. Position Summary:Northwest Habilitation Services is seeking a compassionate and detail-oriented Intake Specialist to join our team. This role is pivotal in welcoming new clients, assessing their eligibility for services, and ensuring a smooth transition into support with IADLS. The ideal candidate will embody our core values of communication, integrity, accountability, professionalism, and commitment to excellence in every interaction. The primary goal of this position is to intake clients with a persistant mental health diagnosis and continue to work with them weekly until ready to transition to othe direct support staff as needed. Key Responsibilities:• Conduct initial onboarding for new clients, including intake assessments and service viability evaluations• Introduce clients to appropriate staff members• Provide support and guidance to clients when assigned staff are unavailable• Maintain accurate and confidential client records• Collaborate with clinical and administrative teams to ensure continuity of care• Uphold safety and de-escalation protocols as needed• Showcase Northwest Habilitation’s core values in all client and team interactions Required Qualifications:• CPI (Crisis Prevention Intervention) Certification• QPR (Question, Persuade, Refer) Certification• Minimum of 2 years experience in a mental health setting• Valid driver’s license and reliable transportation• Strong interpersonal and communication skills• Ability to work independently and as part of a multidisciplinary team Work Hours:• Minimum of 30 hours per week• Opportunity to work up to 40 hours per week based on program needs Why Join Us?This is more than a job, it’s a chance to make a meaningful impact in the lives of individuals seeking support.
