Position Overview: As a Social Worker, you’ll help plan, organize, and direct social services to meet the emotional and social needs of our residents. You’ll play a key role in ensuring each resident’s well-being and supporting their adjustment to life changes. Qualifications: Active social work license in Massachusetts (LSW, LICSW, LSWA, or LCSW). Bachelor’s degree in social work from an accredited school. At least 1 year of experience in a skilled nursing facility Key Responsibilities: Coordinate and monitor resident care conferences. Support residents and families through transitions and crises (illness, disability, loss). Advocate for residents’ personal needs and utilize community resources. Complete comprehensive documentation promptly. Comply with privacy policies and protect resident health information. Support staff with resident-related issues. Perform other duties as assigned.
