We are a 100% in-house facility under new management, and we are focused on strong customer service, smooth admissions, and supportive interdisciplinary teamwork.Responsibilities:• Serve as the primary contact for residents and families regarding care planning and support• Manage the admission and discharge process, including documentation, assessments, and care coordination• Work closely with nursing, therapy, and administration to ensure quality outcomes• Maintain compliance with state and federal regulations• Participate in resident care conferences and provide psychosocial support• Oversee and mentor the Social Services team• Assist with insurance authorizations, community resources, and discharge planning• Maintain accurate records, progress notes, and care plansQualifications:• Social Services Designee (SSD), LSW, or related certification required• Experience in long-term care required• Strong knowledge of Medicare/Medicaid, discharge planning, and psychosocial needs• Excellent communication, time management, and documentation skills• Ability to work professionally with residents, families, and staffWhat We Offer:• Competitive pay• Supportive leadership team• In-house staff, no agency• Opportunity to make a meaningful impact on resident well-beingHow to Apply:Please submit your resume directly through Indeed or apply in person at our facility.
