Summary:Ref#: SHP-LPN-PA-WILK.Profession: Licensed Practical Nurse.Specialty: Long-Term Care / Rehab Nursing (SNF).Job Type: Contract/Per Diem.Start Date: ASAP.Location: Wilkes-Barre, PA.Schedule: 3:00pm - 11:00pm.Shift Type: Evenings.Rate: $30.50/hr.Requirements:Active LPN license in the state of Pennsylvania.Minimum of 1 year of experience in long-term care or rehabilitation preferred.Current CPR/BLS certification.Covid Vaccination Card.Valid photo ID.NIH(ASA).2 Supervisory References.Responsibilities:Provide direct nursing care to residents according to facility protocols and physician orders.Administer medications and treatments accurately and safely.Monitor, assess, and document residents’ health conditions and report any changes to the supervising RN or physician.Assist residents with activities of daily living, such as hygiene, mobility, and nutrition.Maintain accurate records of care provided and medications administered.Support and follow infection control and safety guidelines.Collaborate with RNs, CNAs, and other healthcare staff to ensure coordinated care.
