Job Summary:

As a Nurse Practitioner, you will provide primary care, diagnostic assessments, treatment plans, patient education, and preventative care to patients. You will collaborate with our multidisciplinary healthcare team to ensure patients receive exceptional care across various specialties and services.

Key Responsibilities:

  • Perform comprehensive physical exams and assessments.
  • Order, interpret, and follow-up on diagnostic tests and laboratory results.
  • Diagnose and manage acute and chronic conditions.
  • Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
  • Prescribe medications, therapies, and treatments within the scope of practice.
  • Provide patient education and counseling on disease prevention, wellness, and lifestyle changes.
  • Maintain accurate and up-to-date patient records in the electronic health record (EHR) system.

Qualifications:

  • Master's degree in Nursing from an accredited Nurse Practitioner program.
  • Valid Nurse Practitioner license in the state of Oregon.
  • Current certification as an Adult, Family, or Psychiatric Nurse Practitioner (or other appropriate certifications).
  • Minimum of 2 years of experience as a Nurse Practitioner, preferably in a primary care or specialized setting.
  • Current Basic Life Support (BLS) from the American Heart Association (AHA) or the American Red Cross
  • COVID Vaccinated
  • Recent PPD Test or Chest X-Ray
  • Clear criminal background check(s) & drug screening
  • Reliable transportation & smartphone
