Community Action Partnerships of Western Nebraska (CAPWN) is a non-profit community-based health and human services organization that serves low-income, disadvantaged and those unable to meet their needs through other sources. The agency was established in 1965 and provides services to over 9,000 individuals, children and families in three main areas:
- Community Health Services: WIC, Commodity Supplemental Food, Foster Grandparents, Migrant Seasonal Head Start, and Child Development Center.
- Clinical Health Services: Primary Care, Reproductive Health, Diabetes Management, Immunizations, Migrant Health, Ryan White HIV; Behavioral Health including Counseling Services, Medication Management, Medication Assisted Treatment and Substance Use Counseling and Intensive Outpatient Services and Dental Services.

Job Title: Psychiatric Nurse Practitioner
Reports To: Psychiatrist
Areas of Responsibility: Support the goals, philosophy, and objectives of Community Action Health Center (CAHC). Work collaboratively with the psychiatrist to provide patient assessment and diagnosis, formulate treatment plans, prescribe medications, order laboratory tests, interpret test results, provide patient education, prepare medical records, and refer patients for inpatient care in accordance with statues, regulations and protocols regulating the profession. Respectful of the confidentiality of the patients, co-workers, and clinic.
Major Duties:
- Provide behavioral health patient assessment, diagnosis, and treatment plans in accordance with statutes, regulations and protocols regulating the profession.
- Obtain medical history.
- Complete psychiatric assessments and formulate differential diagnoses based on clinical findings.
- Order, conduct, and interpret pertinent laboratory and diagnostic procedures with appropriate follow through.
- Formulate, document, and implement outcome-based treatment plans.
- Prescribe, monitor, and evaluate psychotropic and related medications.
- Order and interpret diagnostic studies such as lab work.
- Communicate test results and follow-up to client.
- Review medication list, screen for interactions, issue medication prescriptions in accordance with treatment guidelines.
- Assess real or potential health hazards and current health status, including analysis of health behavior related to lifestyle and culture.
- Devise, implement and evaluate plans of care utilizing sound clinical judgments based on assessment of the physical, psychological, emotional, societal, and environmental needs of the patients.
- Promote positive health behaviors and self-care skills through education and counseling.
- Collaborate with physicians, therapists and other health professionals as needed.
- Consult, refer and collaborate with other disciplines involved in the delivery of total patient care.
- Document accurately all assessments, interventions, client/family responses, medication dispensed or prescribed, procedures, treatments, and other data in medical record.
- Utilize the resources of CAHC and the surrounding area community.
- Maintain a clean, safe, and efficient clinic environment for clients, visitors and staff.
- Demonstrate evidence of continuing professional growth by involvement in peer review, the review of protocols and procedures and participate in quality improvement within the clinic.
- Attend CAHC meetings and in-service offerings and participate in assigned committees.
- Maintain current Basic Life Support certification.
- Maintain current CPI training.
- Maintain patient confidentiality according to HIPAA regulations.

Working Conditions:
- Clinic environment includes patient interaction, training, meetings, and community interaction.
- Well-lit, temperature-controlled clinical setting, with standard medical and office equipment available.
- Work is frequently stressful and requires meeting tight deadlines.
- Work in a highly volatile program in a state of change with many interruptions and interactions.
- Must be able to work irregular hours when needed and may include some out-of-town travel.
- Frequent exposure to communicable diseases, toxic substances, medicinal preparations, and other conditions common to a medical environment.
Physical Requirements:
- Varied activities including standing, walking, reaching, bending, lifting.
- Requires full range of body motion including handling and lifting clients, manual and finger dexterity, and eye-hand coordination.
- Requires long periods of sitting but will include standing and walking throughout the day.
- Manual dexterity sufficient to operate office and medical equipment.
- Must regularly talk, hear, and have close vision ability to record and communicate appropriate documents and records.
- Occasionally lifting and carrying up to 25 pounds.
Minimum Qualifications:
Grade: 17
- Licensed to practice as a Psychiatric Mental Health Nurse Practitioner in Nebraska.
- Maintain a current and unencumbered DEA.
- Graduation from accredited master’s level Nurse Practitioner program.
- Behavioral Health Clinical experience preferred.
- Current BLS.
- Maintain and have a valid driver’s license and insured vehicle.
- Establish and maintain a good working relationship with the consumer and provider community.
- Bilingual in English and Spanish preferred.
Grade 18:
- Meets above requirements and:
- Has three or more years of experience in behavioral health OR
- Achieve additional certification pertaining to mental health.
Grade 19:
- Meets above requirements and:
- Has five or more years of experience in behavioral health OR
- Achieve two or more additional certifications mental health.
Grade 20:
- Meets above requirements and:
- Has seven or more years of experience in behavioral health.
