Job Overview:
We are seeking an experienced Intake Coordinator with a background in home health to join our team. As the Intake Coordinator, you will be responsible for managing the initial intake process for new home health patients, ensuring seamless coordination between patients, caregivers, healthcare providers, and administrative staff. Your role will be pivotal in facilitating efficient and effective communication and documentation to initiate patient care services.


Responsibilities:


  1. Patient Intake Management:
    • Receive and process incoming referrals for home health services.
    • Conduct initial assessments to determine patient eligibility and care needs.
    • Coordinate with healthcare professionals to gather necessary medical information and documentation.
    • Verify insurance coverage and obtain authorization for services as required.
  2. Communication and Coordination:
    • Serve as the primary point of contact for patients, families, and healthcare providers during the intake process.
    • Communicate effectively with interdisciplinary team members to ensure seamless transition of care.
    • Coordinate scheduling of initial assessments, visits, and services with patients and caregivers.
    • Provide timely updates and feedback to all stakeholders regarding intake status and any required follow-up actions.
  3. Documentation and Compliance:
    • Maintain accurate and up-to-date electronic health records (EHR) for all intake activities.
    • Ensure compliance with regulatory requirements, including Medicare, Medicaid, and private insurance guidelines.
    • Complete necessary paperwork and documentation for intake, admission, and authorization processes.
    • Collaborate with billing and coding staff to ensure accurate and timely submission of claims.
  4. Quality Assurance:
    • Participate in quality assurance activities to monitor and improve the intake process.
    • Identify opportunities for process optimization and efficiency enhancement.
    • Address patient and caregiver concerns or issues related to the intake process promptly and effectively.

Qualifications:


  • Bachelor's degree in healthcare administration, nursing, social work, or related field preferred.
  • Minimum of 2 years of experience in healthcare, with specific experience in home health intake coordination strongly preferred.
  • Knowledge of home health regulations, reimbursement policies, and documentation requirements.
  • Proficiency in using electronic health record (EHR) systems and Microsoft Office applications.
  • Excellent communication skills, both verbal and written, with the ability to interact effectively with diverse stakeholders.
  • Strong organizational skills and attention to detail, with the ability to prioritize tasks and manage multiple deadlines.
  • Ability to work independently and collaboratively within a team environment.
  • Compassionate and empathetic attitude towards patients and families, with a commitment to delivering high-quality care.
