Job description

We are seeking a dedicated ARNP to join our healthcare team as a contractor. The ideal candidate will possess a strong medical background and a passion for delivering quality and comprehensive inpatient and outpatient care. Must be cognizant of, and comply with, all Center policies and procedures, as well as pertinent state and federal regulations. APRN must provide the full scope of pulmonary care services at nursing homes and long term care facilities, including but not limited to diagnosis, treatment, coordination of care, preventive care and health maintenance of their patients. The company provides Nurse- Practitioners to Skilled Nursing homes in Florida to work with the facilities nursing team to improve quality care and reduce rehospitalizations. The company is currently supporting Skilled Nursing Facilities (SNF) in the Miami, Broward and Tampa area, but other areas are also available.

Responsibilities:

  • Provide direct patient care in various healthcare settings, such as nursing homes and long term care facilities
  • Obtain health history and perform physical examinations
  • Diagnose and direct counseling for each patient on a plan for treatment
  • Prescribe medication in accordance with state statute and professional practice guidelines
  • Maintains a problem-oriented health record for each patient
  • Evaluation of new-admission visits
  • Coordination of discharge visits
  • Develop and implement patient management policies and procedures
  • Communicate with attending physicians and/or specialists regarding patient care
  • Conduct routine check-ups to patients to assess their health condition and discover possible issues
  • Give appropriate advice for healthy habits (diet, hygiene etc.) and preventative actions to promote overall health
  • Cultivate a climate of trust and compassion for patients
  • Assures continuity of care by providing comprehensive medical services on a timely basis
  • Provides excellent internal/external customer service via telephone, videoconference, chat or face-to-face contact to assist patients with their health care needs
  • Follows established departmental and medical center policies, procedures, and objectives
  • Monitor, assess, and document patient conditions and reactions to treatments.Collaborate with healthcare professionals to plan, manage, and assess patient treatments
  • Educate patients and their families on proper healthcare procedures
  • Ensure compliance with infection control policies and procedures
  • Assist in medical procedures as needed

Skills: The ideal candidate should have experience or knowledge in the following areas:

  • Performing physical exams and patient observations
  • Behavioral health assessment
  • Setting up and monitoring medical equipment
  • Communicating test results to patients and their families
  • Recording patient medical histories
  • Family planning services
  • Infection control protocols
  • Post-acute care experience (Preferred)
  • Senior care practices
  • Medical-surgical nursing techniques
  • Strong anatomy knowledge

  • Excellent written and verbal communication skills.
  • Strong leadership experience with emphasis on teamwork in a healthcare setting.
  • BLS Certification
  • ACLS Certification
  • EMR/EHR experience
  • Experience in Skilled Nursing Homes a plus but not mandatory
