Intake Coordinator

Title: Intake Coordinator Veterans Program Reports To: Director of Contracts and Outreach

Salary $19-22/hour Supervises: None

Hours: Part-Time (25 hours/week) OSHA Exposure Category: II

Position Summary:

Individual in this position will be responsible for the coordination of Intakes at Community Outreach Inc. (COI) in the Good2Go (G2G) Veterans Program. Essential job functions include those listed below.

Essential Job Functions:

• Managing an inquiry list and screening candidates for the Good2Go Program.
• Timely and accurate documentation and tracking of referrals.
• Support the Veteran Peer Support position to successfully complete necessary paperwork/forms.
• Coordinate other G2G staff to schedule intakes, tours, and exits
• Attend scheduled meetings within G2G COI.
• Interface with potential residents and community members professionally.
• Determine initial eligibility for our G2G Veterans Program.
• Exercise appropriate judgment with regard to confidentiality and practice HIPAA compliance.
• Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of Community Outreach Inc. and perform any other duties as

Essential Qualifications:

• Two (2) years of human/social service experience or a bachelors degree in related field.
• Excellent communication
• Basic computer
• Good documentation
• Strong organization and experience with process/system development.
• Familiarity with unhoused and military
• Experience with HMIS and Caseworthy preferred.
• Must pass a DHS background.
• Minimum of three years military service active duty preferred.
• Be able to multi-task.

Education:

• HS Diploma, Or GED

Working Conditions:

• Work will primarily be conducted in an office setting, located within a residential housing program.

Physical Demands:

• Occasional lifting of more than 10
• Sitting, standing, walking, reaching are performed in the normal course of the position; and
• The normal work routine involves no exposure to blood, body fluids, or tissues, BUT exposure or potential exposure may be required as a condition of employment. Appropriate personal protective equipment will be readily available to every employee engaged in Category II
