About the Role:

Our client is looking for a compassionate and skilled Nurse Practitioner to join their team in Stockton, CA. In this role, you'll collaborate with Primary Care Physicians, Community-Based Primary Care Physicians, and an interdisciplinary team (IDT) to deliver comprehensive care for our client's participants. You'll have the opportunity to perform in-person assessments, coordinate care plans, and provide essential healthcare services to frail and elderly participants, empowering them to thrive in their communities.

What You'll Do:

  • Conduct primary and acute care evaluations for participants in collaboration with other healthcare providers.
  • Perform comprehensive in-person assessments, both scheduled and unscheduled, and collaborate with the IDT to develop personalized care plans.
  • Manage discharge planning for participants leaving acute or long-term care settings.
  • Coordinate participant care in nursing homes, providing regular visits and overseeing ongoing treatment.
  • Be an active member of the IDT, working with physicians and other care team members to ensure seamless care and 24-hour delivery when needed.
  • Participate in rotating night and weekend call to support participant needs.
  • Ensure timely and thorough documentation to support participant care and team collaboration.

Required Qualifications:

  • Graduate of an Accredited School of Nursing with RN State License
  • Certification as a Nurse Practitioner with Prescriptive Authority
  • Active DEA Registration and eligibility for Medicare and Medi-Cal enrollment
  • Reliable transportation for community-based care

Preferred Experience:

  • Prior experience in PACE (Program of All-Inclusive Care for the Elderly) or similar settings
  • Experience working in managed care and with interdisciplinary teams
