SUMMARY:

The Nurse Practitioner/Physician Assistant will provide quality primary care services within the Rhode Island Department of Corrections. The Nurse Practitioner/Physician Assistant shall be responsible to the clinical physician lead at the site and responsible for overseeing the progress of patients seen in the medical clinic. Acts as an educator, consultant and liaison to healthcare providers caring for incarcerated patients across the care continuum.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Consistently apply the Brown Medicine values of patient care priority, dignity, collaboration, integrity and quality in support of the Brown Medicine mission to deliver compassionate, high-quality patient care, research excellence and outstanding physician education. Is responsible for knowing and acting in accordance with the Brown Medicine Compliance Program and Code of Conduct. Promote the organization’s purpose and principles. Consistently practice the Brown Medicine Customer Service Standards.

Employ advanced skills consistent with training, level of expertise, and accepted standards of practice, and which are authorized by law and regulation. Such skills will include evaluation, diagnosis and treatment of the primary care needs of adults, as well as the issuance of written or verbal medication orders or prescriptions for controlled substances or other medications in accordance with applicable federal and state law. The scope of practice shall include but not be limited to:

  1. Patient history, physical assessment, and evaluation;
  2. Provide health education, including but not limited to, chronic illnesses and infectious diseases. This may include addressing medical, physical, psychological and social needs and providing options for care and treatment with the goal of facilitating the patient to become an informed, active, engaged participant in their care;
  3. Management of routine preventative health care needs as well as episodic and chronic illnesses;
  4. Diagnose, manage and treat common illnesses and acute conditions.
  5. Ordering and interpreting laboratory and other diagnostic tests;
  6. Consultation with medical specialties regarding diagnostic and/or treatment plans;
  7. Referral of patients to medical specialties as appropriate for further diagnosis or treatment;
  8. Establish treatment plans to include health education, counseling, collaboration and follow up to self-management goals;
  9. Initiation and/or revision of medication regimens, including documentation of all such prescriptions;
  10. Provide care coordination and management of chronic conditions using Evidence Based Guidelines;
  11. Attend and actively participate in provider meetings;
  12. Maintain timely and complete documentation of all patient encounters in the EMR in accordance with Federal /State regulations and Brown Medicine/practice established protocols;
  13. Collaborate with the physicians and all members of the care team in the immediate and ongoing care of the patient;
  14. Participate in the ongoing education of staff with the goal of having all members of the team functioning at the highest level of their license in contributing to the care and education of the patient;
  15. Continuing education as required

  • Display flexibility in altering implementation of interventions when new information is added.
  • Communicate effectively in all settings identifying, addressing and resolving challenges. Maintain open channel of communications with patients, families, staff, colleagues and supervisor. Communicate in a direct, truthful manner, with particular sensitivity to working in a correctional health setting
  • Effectively teach patients, staff and other members of the health care team.
  • Demonstrate the ability to anticipate and plan for changes in the environment.
  • Delegate care activities to others based on skill and knowledge level.
  • Ability to identify a problem and determine its cause(s). Formulate, implement and facilitate solutions
  • Pursue formal and informal opportunities to enhance professional status within organization and throughout the community.
  • Remain current in the latest concepts, techniques and methods.
  • Annually completes mandatory safety infection control, BLS, and ACLS.
  • Prescriptive privileges per licensure.

BASIC KNOWLEDGE:

For Nurse Practitioner

  • Masters of Science Degree, Graduate of accredited Nurse Practitioner program
  • Baccalaureate in Nursing
  • Certified in Basic Cardiac Life Support (BLS)
  • Current RI licensure

For Physician Assistant

  • Graduate of a certified Physician Assistant program
  • National Physician Assistant Certification required
  • Certified in Basic Cardiac Life Support (BLS)
  • Current RI licensure

Demonstrates knowledge and skills necessary to provide care to patients with consideration of aging processes, human development stages and cultural patterns in each step of the care process.

EXPERIENCE:

  • Preferred two-three years of primary care practice experience, preferably some in a correctional health setting.
  • Experience with EMR preferred.

WORKING CONDITIONS AND PHYSICAL REQUIREMENTS:

General correctional health medical environment with occasional stressful conditions associated with patient care.
