We are immediately seeking an experienced Registered Nurse (RN)- Home Health Field Nurse to join our team You will be responsible for the OASIS assessments, and treatment of assigned patients residing in the greater South Holland area.

Responsible to: Director of Nursing

Description:

Provides and directs the provision of home nursing care through the competent application of the nursing process.

Qualifications: Associate degree in nursing, BSN and previous home health experience preferred. Current Illinois licensure. Must have reliable transportation

Job hazards: Possible infections from clients, possible needle sticks, possible automobile accidents, possible unsafe neighborhoods

Job Duties / Knowledge

  • Travel within service boundaries
  • Provides services to home health clients through comprehensive nursing skills in accordance with the plan of treatment signed by the physician. Performs initial evaluations, routine and high-tech nursing visits, transfers, resumptions of care after hospitalizations, recertification’s, and discharges according to OASIS and agency guidelines
  • Demonstrates competency in nursing skills including (but not limited to) venipunctures, wound care, infusion therapy and site maintenance, wound care including wound vac, enteral feedings, tracheostomy care, and foley catheter insertion
  • Evaluates and regularly re-evaluates the nursing needs of the client, develops, evaluates, and updates the plan of care. This includes maintaining the clinical record for all patients receiving care.
  • Performs skilled assessments as appropriate to client health care needs and disease process. Communicates changes in patient condition to physician
  • Administer treatments or medications as prescribed by the patient’s physician or podiatrist.
  • Teaches the client and family / caregivers about disease process and management including medication regime, diet, and interventions. Utilizes education material as appropriate
  • Coordinates the plan of care with other disciplines through regular communication about changes, updates, progress to goals, and discharge planning. Attends client care conferences per agency protocol and initiates as needed
  • Demonstrates competency and adherence to standard precautions
  • Makes referrals as appropriate for therapy, Home Health Aides and community resources
