The Care Coordinator serves as the primary point of contact between the agency and referral sources, overseeing the intake and management of patient referrals. This role is key in coordinating daily schedules, assigning patients to the most suitable clinical staff, and ensuring that patient care is both timely and optimized for quality and staff efficiency. Care Coordinators collaborate to support seamless patient care and efficient service by our clinical team.

Schedule:

  • Flexible 8-hour shifts, Monday-Friday, between 8:30 AM and 5:00 PM
  • Rotation of evening on-call (phone calls only) and occasional weekend coverage as needed

Responsibilities:

  • Handle inbound and outbound calls as required
  • Perform a range of clerical support tasks, including data entry and filing
  • Enter demographic information into new patient records accurately
  • Monitor incoming referrals (via fax or from liaison or marketing representatives)
  • Confirm receipt of referrals with the source when necessary
  • Maintain an accurate and up-to-date referral log
  • Verify patient insurance information
  • Ensure clear, effective communication with all parties involved in patient care
  • Keep the patient hospitalization list updated and notify relevant staff as needed
  • Coordinate initial care visits by interacting with patients and their families
  • Compile and organize daily schedules for clinical staff
  • Assign agency clinicians to patients with guidance from the Director of Nursing, Director of Therapy, and Director of Clinical Services
  • Proactively plan client assignments based on location and clinician availability
  • Attend meetings as necessary, which may involve travel

Requirements:

  • High school diploma required; college degree preferred
  • Proficiency in Microsoft Office
  • Strong multi-tasking skills
  • Exceptional telephone and customer service skills
  • Demonstrates patience, flexibility, and a positive, cooperative attitude
  • Valid Michigan driver’s license, auto insurance, and reliable transportation
  • Willingness to participate in weekend and on-call telephone rotations
