We are seeking a Social Worker/ Case Manager to join our team The ideal candidate will provide social work support to frail elderly clients in Brooklyn Full time M-F 9-5. No on-call work. Bilingual in Spanish, Creole a plus, but not necessary. Hybrid schedule 40% remote work, 60% in person.

Responsibilities:

  • Conduct comprehensive needs ( Bio-psychosocial ) assessments, develop and implement care plans based on identified needs. Conduct regular follow-up to ensure that participants' needs are being met. Meet with family members, caregivers, and others who play a key role in the participants' lives as dictated by the care plan.
  • Identify and use appropriate resources to respond to participant needs and make any referrals necessary to implement care plans. Provide follow-up with clients and collateral contacts as indicated.
  • Input data and maintain comprehensive electronic and paper case files, including intakes, care plans, case progress notes, event participation, and units of service provided, and other important documents.
  • Liaise with partners and community groups.
  • Monitor clients via telephone and home visits
  • Access outside resources and/or programs to facilitate all unmet needs of clients
  • Assess nature of elderly clients' situations by interviewing and reviewing personal history and current milieu
  • Develop and execute individual treatment plans
  • Determine appropriate milestones to gauge client progress and determine future needs
  • Communicate client updates to all relevant parties
  • Facilitate referrals to other healthcare professionals and programs
  • Maintain accurate client documentation

Qualifications:

  • (L)MSW

  • Masters level w/ 1 yr experience in human services

  • BSW or BA/BS ( in field closely related to SW)

• Demonstrated case work experience

• Demonstrated experience with aging population, preferred.

• Understanding of the aging process and its impact on individuals physically, socially, mentally, etc.

• Demonstrated experience with working as a member of an interdisciplinary team.

• Experience with volunteer activities a plus.

• Excellent organizational, verbal, written, and interpersonal skills.

• Strong problem solving skills with the ability to effectively handle crisis situations

• Excellent computer skills.

• Demonstrated ability to perform multiple tasks effectively in a fast-paced, challenging, and constantly changing environment.

• Bilingual a plus
