OverviewThe Case Manager II (CM II) is a key member of the primary care interdisciplinary team, providing services for patients with complex care needs. This position conducts patient outreach, engagement, and psychosocial service assessment, assists in developing a patient-centered care plan, leads Enhanced Case Management (ECM), and coordinates service referrals and delivery. The Case Manager meets clients in home, clinic, or community settings as required. The CM II serves specific populations with multiple complex health and social service needs, often providing care outside traditional health center settings, including home visits, hospitals, supportive housing sites, encampments, and shelters.
This is a full-time, grant-funded, benefit-eligible position in west Berkeley.
This position is represented by SEIU-UHW. Salaries and benefits are determined by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of SEIU-UHW, as defined in the CBA.

BenefitsCompensation: $26.60 - $29.07/hour. We offer excellent benefits including medical, dental, vision (including dependent and domestic partner coverage), generous leave benefits including ten paid holidays, Flexible Spending Accounts, and a 403(b) retirement savings plan.

Responsibilities- Outreach to patients who meet case management program eligibility criteria.
- Engage with patients to assess strengths and needs through standard intake and screening tools.
- Involve patients and caregivers in designing and delivering services.
- Facilitate referrals for internal and external resources.
- Maintain a patient caseload according to LifeLong standards.
- Utilize data registries to manage caseload and meet program requirements.
- Provide health education and training to patients.
- Assist patients with accessing public benefits and affordable housing.
- Communicate with patients, care team members, and external partners.
- Maintain knowledge of patients' treatment plans and facilitate service utilization.
- Participate in team meetings to coordinate care.
- Advocate on behalf of patients and support their self-advocacy.
- Provide case management services for patients with complex medical or behavioral health conditions.
- Provide general housing case management services.
- Assist with patient crisis intervention and de-escalation.
- Document patient contacts/services in required data systems.
- Promote diversity, equity, inclusion, and belonging.

- At least two years of relevant work or volunteer experience, or one year as a Case Manager I.
- Proficient in Microsoft Office and database management.
- Access to reliable transportation with current license and insurance.
- Bachelor's Degree in Social Work or related field.
- Lived experience related to homelessness or mental health services.
- Bilingual/Spanish is a plus.
