Job Description

Reports to the Housing Services Program Director

Position Summary

This is a professional position providing individual support, facilitation, advocacy, and housing access services with people who have disabilities. The primary role of the Housing Services Coordinator is to assist people with moving to a home or apartment of their own, in an independent, community/neighborhood-based setting. Employees in this job classification exercise considerable independence and report to and receive direction from the Housing Services Program Director.

Essential Job Functions

Administrative

  1. Adhere to all Arc Northland Policies and Procedures as outlined in Arc Northland’s Employee Handbook.
  2. Must pass Department of Human Services Background Study.
  3. Follow all Department of Human Services Housing Services regulations, including passing a background study.
  4. Maintain necessary records of individuals served, information and referral provided, and all other information required and necessary for effective service delivery and grant reporting.
  5. Turn in required paperwork and reports in a timely fashion.
  6. Accurately track time worked, submit hours and timesheets on time.

Direct Service

  1. Know and understand the variety of Housing Services such as Housing Stabilization Services (HSS), and Housing Access Services (HAS).
  2. Know and understand the Medical Assistance Home Care, ARMHS, PCA or Waiver services.
  3. Provide advocacy for/with individuals as it relates to housing and other situations, as necessary.
  4. Facilitate education/training and growth opportunities within the Housing Services Department.
  5. Evaluate quality of service recipient’s experience at least annually; make modifications as needed.

Other

  1. Work independently.
  2. Work flexible hours.
  3. Have access to reliable transportation.
  4. Other duties as assigned.

Qualifications

  1. A bachelor’s degree in Social Work, Psychology or other Human Services field, preferred, or five plus years direct work experience in Targeted Case Management/ARMHS with a focus on housing.
  2. Experience working with people with disabilities, disability issues and community resources.

Knowledge, Abilities and Skills

  1. Knowledge of disabilities, experience working with people with disabilities, possible needs and community resources.
  2. Understanding of the Person-Centered philosophy, planning and self-determination.
  3. Knowledge and awareness of state and local human service programs.
  4. Knowledge of current issues and legislation that impacts people with disabilities.
  5. Knowledge of computer applications and software.
  6. Ability to communicate effectively both orally and in writing.
  7. Demonstrated ability to use sound judgment in carrying out oral and written instructions.
  8. Ability to establish and maintain successful working relationships with service recipients, fellow teammates, and other disability service providers.
  9. Ability to monitor own schedule and workload, reliably working all paid hours, regularly communicating these to supervisor.
  10. Complete and submit timesheets and necessary documentation on time.
  11. Ability to provide educational opportunities when necessary.
  12. Ability to monitor schedule and workload, communicate changes to the supervisor.
  13. Communicate with supervisor regularly.

Physical/Environmental Demands

  1. This position requires sitting, standing and walking throughout the day.
  2. Weights of objects to be lifted, carried, pushed, or pulled rarely exceed 24 pounds. Simultaneous use of the hands, wrists, and fingers is sometimes required.
  3. Movements such as climbing, stooping, crouching, twisting, bending, reaching, handling, fingering and feeling are sometimes required.
  4. Audio, visual, and verbal functions are vital aspects to performing this position.
  5. Many workdays are spent in the community and may require exposure to outdoor elements including dust, fumes, noise, and glare.

Transportation

This position requires driving to community locations and/or individual homes. Individuals in this position must have access to reliable transportation, a valid driver’s license, current automobile insurance and be able to transport themselves to areas not accessed by public transportation. Occasionally, it may be necessary to transport an individual receiving services.
