Targeted Case Manager

Salary: $35,000
Educational Requirements: Bachelor’s Degree in behavioral science or be a certified alcohol and drug counselor who has a bachelor’s degree in behavioral science. Must have at least (1) year of full-time employment experience working directly with adults in a human service setting.
Duties/Responsibilities: Opportunities for Women will serve women who have past/present domestic violence and/or moderate to severe substance use dependence. This includes case findings, education, and assessing the person in an individual setting and making referrals to other helping agencies. The Case Manager will coordinate relationships among other human services agencies, consult with courts, the Department of Community Based Services (DCBS), domestic violence shelters, law enforcement, and the public in general. The Case Manager will be the initial contact for referrals from community partners and within Pathways. The Case Manager will continually assess the individual needs of their participants and reflect these needs in staff meetings involving various community partners, evaluation, treatment plans, and chart progress notes. The Case Manager will also host a weekly parenting education class using the Nurturing Parenting program. A small part of the Case Manager’s caseload will consist of Kentucky Moms MATR clients – women who are pregnant or postpartum and also have a substance use disorder. Expectations and job duties will be the same as required of Opportunities for Women clients. The Case Manager is responsible for the timely production of reports for client charts and reports required by funding sources, as well as billing documents and time reports in accordance with center policy. This person will be able to demonstrate and consistently practice competency skills, knowledge, and attitude as outlined in Addiction Counseling Competencies: The knowledge, skills, and attitudes of Professional Practice (CSAT TAP #21, 2006). The Case Manager must demonstrate respect and a nonjudgmental attitude toward participants in all contacts with community professionals and agencies. This position requires travel within our service region, home visits with participants, and attendance at quarterly meetings scheduled by the funding source. The Case Manager will be available for additional duties as assigned by his/her supervisor.
Skills/Knowledge: Must successfully complete DBH approved curriculum of targeted case management training within six (6) months of employment and maintain continuing education hours for certification. Must have at least one (1) year of full-time employment experience working directly with adults in a human service setting.
This is a full-time position.
