The position pays $27/hour with a compensation range of $27/hour - $30.19/hour.

Must have Covid-19 Vaccine and Booster.

Role Overview:

This role involves home visitations in Orange County. The Family Support Counselor implements a community-based prevention effort for families with children 0-3 using the Parents As Teachers curriculum. Responsibilities include in-home and office support, parent education classes, case management, and resource assistance.

Job Responsibilities:Prevention Efforts
  • Work with families to provide support and education using Parents As Teachers curriculum.
  • Conduct group parenting classes.
  • Provide assessments and screenings.
  • Assist families in crisis with referrals and case management.
  • Advocate for clients in systems like Cal-Works and Medi-Cal.
  • Translate materials as needed.
  • Participate in community and agency meetings.
Case Management
  • Meet service provision requirements of the Targeted Case Management program.
  • Conduct and document needs assessments.
  • Develop Individualized Service Plans.
  • Linkage, Consultation, and Referral with follow-up.
Documentation
  • Maintain electronic files and meet documentation requirements for the program.
Community Engagement
  • Maintain a high profile in the community.
  • Develop and maintain relationships with community resources.
  • Assist in developing and implementing outreach programs.
General Duties
  • Maintain confidentiality and positive communication within the agency.
  • Exercise good judgment in duties and responsibilities.
Job Qualifications:
  • Bachelor's degree in relevant field or equivalent experience.
  • Experience with children, families, and community outreach.
  • Knowledge of child development and community resources.
  • Bilingual English/Spanish required.
  • Good organizational and communication skills.
