Transportation Coordinator

Status: Full-Time

Wage: Starting at $24/hour

Location: Avamere Rehab of Shoreline; 1250 NE 145th St, Shoreline, WA 98155

The primary responsibility of this position is the coordinating of transportation for referrals and doctors appointments.

We are committed to providing exceptional care to our residents. As part of our team, you’ll play an integral role in ensuring timely and safe transportation for residents to medical appointments, activities, and other essential needs.

Position Summary

The Transportation Coordinator/Scheduler oversees and coordinates transportation services for residents. This role ensures that all appointments are scheduled efficiently and transportation is provided in a safe, timely, and organized manner. Position will also help medical records with scanning and other miscellaneous administrative duties

Qualifications

  • Experience working in a Skilled Nursing Facility (SNF), Assisted Living, or healthcare environment.
  • High School Diploma or equivalent.
  • Must be a licensed certified nursing assistant in this state
  • Must present a professional appearance and reputation to represent Avamere and this facility.
  • Maintain confidentiality of all resident care information in accordance with HIPAA guidelines.

Responsibilities

  • Coordinate and schedule transportation for residents to medical appointments, therapist and outside activities.
  • Develop and maintain a detailed schedule/calendar for drivers and transportation vehicles
  • Communicate appointment times and schedules with residents, families, facility staff and drivers.
  • Collaborate with nursing, social services, and admissions teams to prioritize transportation needs.
  • This role works alongside our care team in transporting our new patients to our facility from local hospital settings.
  • Maintain accurate, up to date records on trip sheets, fuel purchases, incident and accident reports, and vehicle condition reports.
  • Being available as a chaperone with patients during transportation as needed.
