Community Health Planning Consultant

Live Well Springfield: Convened by the Public Health Institute of Western Massachusetts, the Live Well Springfield Coalition brings together over 30 organizations committed to creating and sustaining a healthy culture in Springfield. Past initiatives have addressed areas such as healthy eating, active living, the built environment, economic opportunity, and education. Currently, the coalition is focused on critical issues including housing, environmental justice, age- and dementia-friendly communities, climate change, health, and equity. This initiative unites cross-sector stakeholders dedicated to collaboration. Utilizing a policy, systems, and environmental change framework, the coalition has made significant progress locally and statewide, fostering ongoing health and well-being for all residents.
The Community Health Planning Consultant is responsible for planning, program development, and evaluation of Public Health Institute of Western MA/Partners for a Healthier Community (PHIWM) projects. The Consultant supports subcontractors and community partners in these efforts, cultivating strategic partnerships between local, regional, and state coalitions, nonprofits, businesses, and health, social, and educational entities.
The Consultant's role involves providing strategic thinking and program planning, assisting with project evaluations, and executing the organization’s strategic objectives. Key responsibilities include convening stakeholders, offering technical assistance, using data to drive new initiatives, evaluating program results, and communicating progress.
Job Responsibilities:Program DevelopmentCreate and implement programs promoting best practices in priority health areas. Develop collaborative strategies with community partners, residents, and organizations such as social and faith-based groups.
Community CollaborationFacilitate local collaborations to advance community initiatives. Support grant-developed programs with partners, including data analysis and evaluation. Lead demonstration projects that foster collaborative approaches and strategies for change.
Consultation & FacilitationProvide consultation to partners on managing program start-up processes. Lead action planning to address priority areas, including timelines, milestones, and evaluation measures. Offer leadership and expertise on partnership development, meeting facilitation, and conflict management.
Program EvaluationCollaborate with evaluators to develop models for measuring program performance, outcomes, and community impact. Provide strategic insight on sustaining health initiatives and fostering collaboration among public health leaders.
Project LeadershipUnder the general supervision of the Director of Coalition Building and Community Engagement, the Consultant manages daily activities, staff, programming, and funding responsibilities for a selected program.
Strategic PlanningEnsure community programs align with the corporation’s strategic goals. Use community health planning methodologies to develop high-quality programs, research, assessments, and evaluations.
Continuous ImprovementConduct regular assessments and publish reports on internal and external factors affecting health priority areas. Provide up-to-date, accurate data on performance measures and community indicators to drive long-term change and accountability.
Required Work Experience:- 5 years of relevant experience in a leadership role.
- 5 years of experience equivalent to a Program Manager for a major public health or related program.
- Strong interpersonal, facilitation, and collaborative planning skills.
- Proven teamwork abilities.
- Excellent written and oral communication.
- High computer literacy.
- Demonstrated community problem-solving skills.
- Experience in facilitative leadership in community settings.
- In-depth understanding of public health and healthcare environments, including youth mental health, substance abuse, and violence prevention.
