The Position:

We're seeking an experienced and compassionate Discharge Planner (RN) to coordinate discharge planning with residents/guests, their families, and attending providers while ensuring a smooth transition to the next phase of care. Responsibilities include:

  • Coordinate and manage discharge planning activities in compliance with federal and state regulations.
  • Meet with residents/guests and their families within 72 hours of admission to identify discharge goals and preferences.
  • Collaborate with the healthcare team—including providers, licensed nurses, therapists, and social workers—to develop and implement comprehensive discharge plans.
  • Facilitate timely referrals to external facilities, providers, and community resources to ensure continuity of care.
  • Assist in arranging home health services and necessary medical equipment to support a safe and efficient discharge process.
  • Educating residents/guests on medication management, treatment or wound care, and other necessary nursing care for a safe transition.
  • Monitor residents/guests progress throughout their stay, adjusting discharge plans as needed to meet evolving clinical and social needs.

Requirements:

  • Graduate of an accredited school of nursing.
  • Current license as a Registered Nurse in the State of Hawaii required.
  • 4 or more years’ experience in restorative and gerontological nursing.
  • Discharge planning experience, preferred.
  • Strong communication and interpersonal skills preferred.
  • Ability to effectively multitask and coordinate care in a fast-paced environment preferred.
