Case Manager I

The staff member in this position performs advanced level, specialized case management services for older and/or disabled adults in need of community based long term care services. Positions at this level perform their work and solve problems within the policies and objectives for a specific functional area or program and will devise and set procedures for others. This position reports to the Case Management Supervisor.
Essential Functions and Responsibilities- Provides Case Management to vulnerable adults at risk for institutionalization as prescribed by Washington Administrative Codes for the Title XIX program, complying with program regulations and policy directives.
- Provides comprehensive assessments, using the electronic version of the CARE assessment in the client's home, including completion of service authorizations through ProviderOne.
- Develops and implements care service plans, coordinating with DSHS staff, the ALTCEW consulting nurse and service providers; maintain and coordinate revisions to the plans. Maintain good working relationships.
- Provides oversight of the care being provided to the client and ensures paid caregivers receive authorized payment for personal care services, per Washington Administrative Codes.
- Prepares case file documentation and/or reports.
- Serves as a mandated reporter of vulnerable adult abuse, neglect, and exploitation. Evaluates client safety concerns and actively coordinates with APS, CPS, and law enforcement on investigations.
- Provides termination planning and Administrative hearing advisement.
- Participates in the Administrative Hearing process as needed. May testify as a Department witness and give legal depositions at Administrative Hearings.
- Provides case management services to facilitate out-of-home placements.
- Provides supportive functions such as client advocacy, assistance, networking and family support, becoming thoroughly acquainted with community resources available for individuals needing long term care.
- Maintains client service records, including electronic records, in a complete and timely manner.
Ability to drive to home visits as needed using personal vehicle. Driving demands average 150 to 300 miles per month depending on client needs. Ability to work on computers and the telephone for long stretches of the day, frequent typing for up to 5 hours per day. Speech, visual, and hearing skills sufficient to interact with staff and the public. Finger dexterity to operate computers. Occasional periods of writing. Infrequent bending, stretching and lifting. Ability to lift 15 pounds on a regular basis (computer case with laptop and files on home visits).
Skills, Knowledge and AbilitiesKnowledge of aging and long term care services including the Area Agency on Aging service network. Ability to learn and apply federal and state policy directives and program regulations. Ability to apply observation and analytical techniques in evaluating situations and formulating conclusions. Knowledge of federal, state and local codes and regulations pertaining to community based in-home personal care case management.
Proficient with Word and Excel. Ability to perform data entries with speed and accuracy and to quickly learn software platforms that change to support delivery systems. Ability to be supervised.

Preferred QualificationsA Masters degree in behavior or health science and one year paid on-the-job social service experience; or Bachelors degree in behavioral or health sciences and two years of paid on-the-job social services experience; or Bachelors degree and four years of paid on-the-job social service experience.
Minimum QualificationsAt least two years of college level courses in a relevant field and two years of paid on-the-job social service experience. Bilingual or bicultural candidates encouraged to apply.
This position requires a criminal background check that qualifies the individual to work with vulnerable adults (WAC 388-133). Must have and maintain a valid drivers license. Employees driving a privately owned vehicle while on agency business must have liability insurance on the privately owned vehicle.
