We are hiring Homecare RN or LPN's Per Diem (weekends required) in Lowell/Greater Lowell and the surrounding areas. We care for mental health patients of all ages and backgrounds who require daily visits in their homes.

Position 1: RN or LPN Per Diem to work Weekends, Weekdays AM and PM shifts in Lowell/Greater Lowell and the surrounding areas, up to $35/visit.

Requirements For This RN Or LPN Job

To thrive as a Home Health Nurse with Comfort Home Care, you'll need a blend of clinical expertise and exceptional interpersonal skills. You'll be responsible for conducting initial and ongoing assessments to accurately identify each client's medical needs, level of functioning, and communication abilities. Familiarity with software for OASIS assessments and timely documentation is crucial, enabling you to initiate and revise care plans effectively. A knack for coordinating services and collaborating with patients, families, and healthcare providers is essential in ensuring seamless care transitions.

Your ability to communicate changes in client conditions and provide nursing interventions will make a significant difference in their lives. Lastly, organizational skills are vital as you'll be preparing clinical and progress notes while adhering to agency policies. Knowledge and skills required for the position are:

Comfort Home Care is seeking a Massachusetts licensed RN/LPN that can:

  • Provides initial and ongoing assessments to identify the client's medical needs level of functioning and ability to communicate and learn.
  • Initiates the plan of care necessary revisions and performs OASIS assessments at appropriate time points when applicable.
  • Initiates and performs appropriate nursing interventions.
  • Coordinates services informs the physician and other personnel of changes in the client's condition and needs.
  • Collaborates with patient /family and other health care providers and/or community resources with planning of care and discharge.
  • Prepares clinical and progress notes and submits all required paperwork in a timely manner according to agency policy and procedure.
