Home Health Aide

The Home Health Aide is responsible for delivering personal care services to provide and maintain bodily and emotional comfort, and to assist patients through the dying process in a safe environment under the supervision of the assigned professional staff. All tasks will be assigned within the scope of each individual’s education, experience, and capabilities.
Essential Duties and ResponsibilitiesThese duties and responsibilities described below represent the general tasks performed on a daily basis; other tasks may be assigned.
- Review the Aide Care Plan every time the patient is visited
- Assist the patient with bathing activities, including: tub and/or shower bathing, oral hygiene, skin care, and hair care
- Assist patient to and from the bathroom, and assist in using bedpans or toilet as needed
- Assist patients in and out of bed and/or chair, and with ambulation
- Assist patient with prescribed exercises as instructed by the appropriate healthcare professional
- Perform incidental household services as are essential to patient care, i.e. light dusting, meal prep, clearing floor for safe patient mobility, etc.
- Report to professional staff and PCC any changes in the patient’s condition or family situation/dynamics
- Measure and record intake and output
- Change linens
- Assess and record vital signs, including blood pressure
- Provide and/or assist with post-mortem care
- Assist patient by handling their pre-poured medication or bottle as instructed by the nurse
- Administer soapsuds or pre-measured enema as ordered by a physician; an RN must administer the initial enema for each client, and must supervise the Aide performing additional enemas
- Administer oxygen by cannula or mask
- Apply ice bags or hot water bottles as ordered
- Check for and remove fecal impaction and/or empty colostomy bags as ordered by the physician and with RN supervision
- Check rectal temperature
- Utilize a Hoyer Lift as necessary to transfer patient to and from shower/tub with RN supervision
- Transfer patients post-joint replacement surgery with instruction from the Physical Therapist
- Perform simple dressings as specifically ordered by the MD or RN
- Wash hands before and after each visit
- Comply with agency bag technique
- Wear gloves appropriately
- Follow OSHA guidelines for infection control measures and universal precautions
- Review policy and procedure manual and HHA job description periodically
- Notify the HOC/Nurse Manager within 48 hours of missed visit
- Ensure care is provided according to HHA plan of care and frequency is maintained
- Comply with documentation of progress notes
- Respond to corrective action reports within one week
- Ensure documentation is completed thoroughly, accurately, legibly, and in a timely manner
- Complete QCC reports for unusual occurrences within 24 hours and submit to PCC; notify HOC of incident upon occurrence
- Organize workload efficiently and prioritize duties for effective time management
- Assume extra patients as needed; respond well to unexpected changes in work volume
- Attend required hospital-wide orientations, meetings, and in-services
- Demonstrate a commitment to flexible work scheduling when necessary to ensure patient care
Minimum Required Education: High school diploma or GED, and successful completion of a Home Health Aide program
Experience: Prior training and experience in an accredited hospice facility, or another accredited, comparable institution is required
Certificates, Licenses, RegistrationsApplicant must be licensed to work in the Commonwealth of Virginia as a Home Health Aide and must be BLS certified through the American Heart Association by start of employment. Applicant must have a valid, unobstructed driver’s license, a reliable source of transportation, and a working telephone.
