Position Summary:
The Immigrant and Refugee Health Center (IRHC) Case Manager is responsible for supporting patients of the IRHC, including those in its specialty programs such as Psychiatry (Boston Center for Refugee Health and Human Rights), General Internal Medicine, OB/GYN, and collaborating departments throughout BMC. The role focuses on patient experience and improving health from a biopsychosocial perspective.
The case manager conducts needs assessments and mental health acuity assessments for new patients, serves as a key member of the IRHC Case Management Team, and provides case management services that involve identifying patient strengths and barriers and facilitating appropriate referrals. They ensure compliance with data collection and collaborate closely with clinical and administrative staff.
Career development opportunities include involvement in research and academic initiatives, educational opportunities, and training hospital staff. The ideal candidate will have strong interpersonal and critical thinking skills, experience with vulnerable populations, and preference will be given to bilingual candidates with connections to immigrant communities.
Job RequirementsEducation:- Masters’ degree in social work (MSW) or counseling preferred, or Bachelor’s degree in social work or a related field with at least 3 years of relevant experience.
- Experience with mental health populations and ability to triage mental health crises.
- Experience with immigrant and refugee populations, preferably with lived experience as an immigrant in the U.S.
- Comfort with risk assessment and triaging required.
- Familiarity with electronic medical record systems.
- Ability to multitask and adapt to changes in healthcare delivery.
- Excellent communication skills and ability to work with diverse populations.
- Strong organizational and time management skills.
- Familiarity with trauma-informed approaches and knowledge of resources for immigrant populations.
Conduct Immigrant and Refugee Health Center Needs Assessment/Intake for new clients:
- Mental health acuity/crisis assessment and facilitation of appropriate triaging.
- Comprehensive assessment of client’s medical, mental health, and social needs.
- Collaborate with administrative staff to schedule client appointments.
- Document needs assessment data in EMR.
Serve as a key member of the IRHC Case Management Team:
- Assess acute and high-level biopsychosocial needs; connect clients with appropriate resources.
- Participate in developing and refining case management processes.
Provide case management for established IRHC clients:
- Create a welcoming environment and encourage engagement in care.
- Connect clients to services including mental health, primary care, and legal support.
- Facilitate interdisciplinary consultations and maintain documentation.
Program development and administrative responsibilities include:
- Participate in team meetings and quality improvement initiatives.
- Engage in learning opportunities and adhere to organizational policies.
